[OPS-MAINT] Standing Outcome: unplanned fixes and hygiene #5097

Root/fdev (Programme/root).
Purpose
This is the standing Outcome for unplanned fixes and hygiene that belong to no other Outcome. Examples: an emergency fix to main, a broken scheduled check, a stale census hash, a flaky test.
Under the dispatch rule (#5092), every such fix still gets its own Delivery issue, filed here as a native sub-issue with a one-line body: owner, done-when, and Refs.
Done when
This Outcome is never closed. It is a standing container. Its Deliveries are closed individually on acceptance.
Intended fields at enrollment: Work level Outcome; Track Follow-up; Status In progress.
