Six of seven tools only called a finished turn 'waiting' when the last message held a question mark, which caught 24% of real turn endings. Every monitor already reports the state that matters: the turn is done and the process is still open. After 60 seconds in that state with no new activity, post 'waiting on you' once for that turn, for every tool, local and remote. Turns that ended before launch, closed processes, dismissed turns and turns already announced by the question rule are skipped. Approval banners now say 'needs your approval', since AskUserQuestion arrives through the permission hook with the question as the body. …esses Codex 0.157 writes bulk records the parser ignores. When the last 64KB held only those, the session parsed to nothing and discovery dropped it, deleting its row and alert history. Replayed against a real 48MB rollout that was about one poll in seven to ten; reading further back (1MB, then the 4MB ceiling) when the window has nothing readable brings it to 0.2%. Kimi Code 2.0 renamed its process from kimi to kimi-code. Liveness only counted 'kimi', so every live 2.0 session read as closed, and the pid lookup behind Approve and Deny could not find the terminal.
