fix: fail closed on unreadable Claude session directories - #23
Keep an unreadable Claude session directory from authorizing a profile mutation or token refresh.
The scanner now distinguishes a missing directory from a failed enumeration. Its callers preserve that uncertainty through backup updates, refresh, mutation guards, and purge.
Changes
Enumerate session records inside explicit error handling; preserve missing-directory behavior and platform-specific JSON filename matching.
Remove the outer existence shortcut so the scanner determines whether state is absent or unreadable.
Preserve profile credentials and use the existing stale marker after a backup update when sessions are live or unreadable. A marker failure is logged without misreporting a successful backup write.
Defer backup-token refresh when session state is unreadable, before adopting or consuming the profile's token generation. Preserve the session-unreadable reason through usage and autoswitch, without an unnecessary request using a known-expired token.
Evaluate live sessions and unreadable state from one scan in the mutation guard.
Inspect the purge session root and each child with explicit error handling; refuse stat or enumeration errors before confirmation or deletion. Tolerate children that have disappeared.
Keep read-only session listing tolerant and provide short recovery guidance.
No new dependencies, token formats, or Codex behavior changes. CHANGELOG.md remains removed as requested.
Follow-up review
I missed several existing callers in the initial review. Some discarded the unreadable count, purge could suppress inspection errors before reaching the scanner, and the mutation guard combined partial results from two scans. The original tests and green CI did not cover those paths. I am sorry for the incomplete review; the first version did not provide end-to-end protection across these operations.
This update fixes those paths and adds regressions for them, including both purge root and child failures, partial enumeration failure, failed stale marking, and safe normal behavior.

Walkthrough
Session scanning now reports directory-enumeration errors as unreadable state. Live-session checks use that result, and mutation guards refuse actions when the scan cannot confirm whether Claude is running. The error message and session-mode documentation describe this behavior.
scan_sessions distinguishes a missing directory from other enumeration errors. scan_live_sessions delegates to it without checking directory existence first. Tests cover error handling and filename matching.
Apply uncertainty to mutation checks tests/test_session_scan_errors.py, src/claude_swap/switcher.py, docs/session-mode.md
Tests cover non-quiescent profiles, mutation refusal, and tolerant read-only listings. The error message and documentation describe the response when session records cannot be read.
Unreadable session state can still lead to destructive credential or profile removal while Claude is running. Both mutation paths should fail closed before merging.

Keep scan uncertainty in the backup-write guard.
When the profile’s sessions directory cannot be enumerated, scan_live_sessions() returns ([], 1). _live_session_pids() drops the unreadable count, so this condition takes the else branch. A normal backup-credential update then calls _invalidate_session_credentials(), which removes the profile’s credential file and, on macOS, its Keychain entry while Claude may still be running. Check the unreadable count here. If the scan is uncertain, mark the profile stale instead of invalidating its credentials.

Update the backup-write guard that calls _live_session_pids to retain the

mark the profile stale and skip _invalidate_session_credentials, while
preserving normal invalidation when the scan is complete and no live sessions

Nice catch, thanks. Treating a failed scan of sessions/ as "nothing running" is exactly the wrong way to fail, and the fix is small and in the right place. iterdir() inside the try, FileNotFoundError still meaning "no records", and anything else counting as unreadable all look right to me. Dropping the exists() shortcut is fine too, since a missing profile now just lands on FileNotFoundError. The tests that inject at listdir/scandir are a good call for CI.
The new section in docs/session-mode.md can be a lot shorter. Something like "If the sessions folder can't be read, ccswap can't tell whether Claude is running, so it refuses the action. Restore access and retry." covers it. The bits about the internal unreadable count and Codex not changing are not things a user needs.
CI is green, so once those are in I will merge it.

src/claude_swap/switcher.py:7855-7858 🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win
Reject unreadable session roots before purge mutation.
Python 3.14 makes Path.is_dir() return False for any OSError. When backup_dir/sessions cannot be inspected, this code treats it as absent and skips all live-session and unreadable checks.
After confirmation, purge removes credentials and calls shutil.rmtree(self.backup_dir). It can therefore remove session profiles without proving that no Claude process uses them. If removal also fails, credentials were already deleted.
Handle root inspection with stat() and iterdir(). Treat only a missing or non-directory root as absent. Refuse other filesystem errors before mutation.

I went back through the session-state callers and found gaps I should have caught in the first review. I am sorry for presenting the scanner fix without tracing all the mutation and token-refresh paths that depend on it.
The first version fixed directory enumeration, but:
backup invalidation and profile-token adoption still discarded the unreadable count;
purge could skip the session root or individual profiles when is_dir() suppressed a filesystem error;
the mutation guard checked PIDs from one scan and unreadable state from another, so neither result was evaluated completely.
These bugs were already present in the base branch, but that does not excuse missing them in this PR's review. Green CI and the earlier tests did not establish that those paths were safe.
The update preserves credentials and marks the profile stale when session state is unknown, defers token refresh, checks both parts of one scan in the guard, and explicitly handles root, child, and enumeration errors before purge can reach confirmation or deletion. The purge fix covers individual child inspection as well as the root, while tolerating children that have disappeared. The refresh refusal keeps its own actionable reason in usage and autoswitch instead of appearing as network trouble or triggering a doomed request with an expired token.
The first set of added regressions fails on the previous head (8 failed, 7 passed) and passes with the corrections. The final full Linux suite passes on Python 3.12 and 3.13 (4491 passed, 9 skipped each); the targeted session, OAuth and autoswitch suites also pass on Python 3.14 (619 passed). The new tests also cover a failed stale-marker write and the normal idle-profile paths. The documentation stays short, and CHANGELOG.md stays removed as requested.
An independent read-only security review found two smaller follow-ups (the disappeared-child case and the misleading refresh status); both were corrected and re-reviewed, with no remaining findings in the examined paths. Test execution was separate from that review.
CI for commit 6c7b831 is now green on Linux, Windows and macOS: run. CodeRabbit reports review rate limited for this push, so I am not treating its green status as a completed review of the new code.
