fix(search): limit same_entity recency marker to diff/doc rows

Closes #269

変更内容

• markRecency（src/aggregate.ts）が各行の type を受け取り、diff / doc 行のみで updated_at を比較するよう変更。その他の type には marker を付けず、比較にも参加させない。
• src/mcp.ts の呼び出しで type を渡すよう変更し、ツール説明文の recency の文を diff/doc 行限定に更新。
• テスト追加: issue + コメント群で marker が付かないこと、混在グループで diff/doc のみ比較されること。
