fix(search): limit recency marker to file-version rows (diff/doc)

purpose

#267（PR #268）で入れた recency 印が、版の関係にない行にまで付く誤りを直す。

premise

• 2026-10-03 10:35Z の本番検索「webhook ownership test own external events」（repo=Liplus-Project/liplus-language）で、issue #2007 の本文行が recency=latest、同じ実体に畳まれたコメント 4 件が recency=superseded になった。#1890、#1997 も同じ形。
• same_entity は issue/PR とそのコメント・レビューを一つの実体として畳む（src/mcp.ts ツール説明「an issue or PR and its comments / reviews are one result」）。feat(search): add recency marker to same_entity rows #268 の markRecency は種類を区別せず updated_at だけで比べている（src/aggregate.ts markRecency）。
• コメントは本文の旧版ではないので、本文の更新日時が新しいことを理由に「この後に新しい変更あり」とするのは誤り。

constraints

• 印の比較対象は、同じファイルの版の関係にある行（type が diff と doc）に限る。issue / pull_request / issue_comment / pr_review / pr_review_comment / release / wiki_doc の行には付けない。
• 順位・score・rerank・graph は変えない。
• ツール説明文の recency の記述を、対象が差分と doc に限られることが分かる形に直す。
• テストに「issue 本文 + コメント」の組で印が付かないケースを足す。

completion condition (PR 単独では満たせない)

• デプロイ後、上記の本番検索で #2007 のコメントに recency が付かず、かつ「subagent model effort selection criteria spawn」で c6f16c3 の差分に superseded が付いたままであることを確認する。

完了条件を確認（2026-10-03 10:39Z、本番検索）: 「webhook ownership test own external events」で #2007・#1890 の issue 行とコメントに recency が付かなくなった。同じ結果の 51623e2 の差分には superseded、31dcfb2 に latest が残っている。
