fix(file): stream the Items array of a DynamoDB scan dump - #217
The output of aws dynamodb scan is one JSON object that holds the whole table in its "Items" array. The DynamoDB dump reader decoded that object whole into a map[string]json.RawMessage, then unmarshalled the array, so a scan dump stayed in memory several times over before the first record went out. A driver review for unbounded reads found it. AGENTS.md asks that large results stream.
Change
The reader walks each top-level object key by key with json.Decoder.Token.
On "Items" it consumes [ and decodes one item at a time, checks the context per item, and consumes ].
On "Item" (one line of an S3 export) it decodes the one item as before.
Other keys (Count, ScannedCount, LastEvaluatedKey, and so on) are read and dropped.
The records do not change. An S3 export with one "Item" per line, and several scan objects in one file, still work. The errors keep their messages: an object with neither key, malformed JSON, and an "Items" that is not an array.
Behavior changes for inputs that a real dump never has: an object with both "Items" and "Item" now emits both (before, "Items" won), and a syntax error inside one item now reads decode dynamodb dump instead of decode dynamodb Items.

Tests added: TestDynamoScanOutputStreams (20,000 items through a byte-counting reader; the first record must arrive before 10% of the input is read; it fails on the old code with 1728936 bytes read of 1728936), and TestDynamoSourceShapes (scan output, two scan objects, an S3 export, extra keys around Items, an empty Items, and the error cases). TestDynamoSourceShapes also passes on the old code, so the output is unchanged.

The output of `aws dynamodb scan` is one object that holds the whole
table in its "Items" array. The reader decoded that object whole, then
the array, so it kept the whole file in memory several times over.
Walk each object key by key with json.Decoder.Token. Decode the "Items"
array one item at a time, decode an "Item" line as before, and skip the
other keys. The records do not change, and an S3 export (one "Item" per
line) and several scan objects in one file still work. An object with
both "Items" and "Item" now emits both. A real dump never has both.

Walkthrough
The DynamoDB dump reader now parses sequential objects with JSON tokens and emits array items individually. The tests cover supported input shapes, malformed input, early emission, and cancellation.
The reader parses sequential objects and dispatches Item and Items keys as they appear. Tests cover scan output, concatenated objects, NDJSON exports, empty item lists, and parsing errors.
Streaming items and cancellation drivers/file/dynamodb.go, drivers/file/dynamodb_test.go, mutago-baseline.json, mutago-baseline.notes.md
The reader emits array items one at a time and checks cancellation while reading. Tests check early emission and cancellation. The mutation baseline records three accepted mutants and their justifications.
zsltg/iq#183: Both changes update DynamoDB dump reader handling of end-of-input errors.

The reader’s current guards and cancellation return remain intact, but the tests and mutation baseline do not adequately protect those behaviors. Add the targeted tests and correct the baseline before merging.

CodeScene found readObject and readItems too complex. Move the opening
of an object, the member loop, the element loop and the delimiter check
into small helpers. The behavior does not change.

The mutation gate and the patch coverage found error paths with no
test. Pin the exact error of each broken input and its cause, and test
a cancel in the Items loop.
A json.Decoder keeps its first error, so a dropped guard gave the same
message as the next call. The errors now name the failing part: "decode
dynamodb key" for a bad member name and "decode dynamodb value" for a
bad member value.
Three mutants are equivalent and go in the baseline: the guards on the
Tokens that consume the closing brace and bracket, which Go 1.27 More
makes unreachable, and the context check in the Items loop, which the
outer loop repeats.

CodeRabbit showed that three mutants in the baseline are not
equivalent. More reports false at either closing delimiter, so a
mismatched one reaches the closing guards, and a cancel can be told
apart when the rest of the Items array is broken.
Add rows for an object closed by a bracket, an Items array closed by a
brace, and an object with both Item and Items. Add a cancel test with a
broken second item. The error for the closing brace now reads "decode
dynamodb object end". Remove the three baseline entries. The reader
now has no surviving mutant.
