Incremental generation with file-hash cache - #78

On large repos, re-running `llmstxt-gen generate` parses every file from scratch even when nothing changed. This change introduces a content-hash cache to skip unchanged files. - Added `cache_path` to `LlmsTxtConfig`. - Created `src/llmstxt_gen/cache.py` for hashing, persistence, and serialization. - Updated `src/llmstxt_gen/cli.py` with `--incremental` and `--no-cache` flags. - Refactored `_collect_modules` to use the cache when incremental mode is active. - Added `.llmstxt_cache.json` to `.gitignore`. - Added tests for caching logic and incremental CLI behavior in `tests/test_cache.py`.
