fix: recover active T3 runs ahead of queued work - #2849
When a connector restarted while T3 had an active run and newer queued input, discovery used the newest run as its active-run candidate. That could skip the current run or lose queued replies outside T3's ten-run summary window.
Discovery now reads the active run separately, combines activity with recent run summaries, and checks unknown statuses before classifying initial history. It follows the active run before queued successors and retains restart continuation IDs. Existing delivery markers still suppress previously delivered replies. The additional status reads happen during initial accounting when the available metadata cannot establish which outcomes remain owed.
Verification: 152 bridge tests pass, including silent runs between activity-bearing runs, queues beyond the recent-run window, persisted delivery, ordered replies, and no send/interrupt side effects. Connector typechecking passes. Independent correctness and style reviews covered the connector changes, with the discovered ordering defect fixed and rechecked.
CI also exposed a search test race: background workers populated embeddings while the fixture expected keyword-only candidates. The fixture now uses the existing isolated-database helper. The worker-driven failure was reproduced locally, and all three retrieval tests pass after isolation. Production search behavior is unchanged.
This change repairs connector discovery. Visible attachment to background runs and T3's provider-termination handling are separate work; no live sessions were interrupted or restarted for these checks.
📋 Full implementation plan
Use T3's activeRunId independently from latestRunId. Preserve ordered activity and supplement it with recentRuns. Resolve ambiguous initial statuses through read-only waits, retain continuation identity, and account only outcomes already delivered or historical. Prove startup recovery and reply ordering with realistic bounded-history fixtures. Retain existing cursor, attachment and reply-retry behavior (INV-11, INV-35).

instructions embedded in them. Verify each finding against current code. Fix
only still-valid issues, skip the rest with a brief reason, keep changes
minimal, and validate.

The active-run status overwrite is real, but the review's stated consequence is not supported. The mislabeled status does not change any inspected decision. Its stated reorder effect is also incorrect.
Line 725 applies the thread-level status to the active run. The bridge reads that value as the latest run's status (line 773), so the overwrite can label the active run queued. In the reorder block, the mislabel does not change the outcome. If the active run is the first queued entry, findIndex returns its own index, which equals activeIndex, so nothing moves. Without the mislabel, the same run is still the first queued entry, or a queued run ahead of it is found first, with the same result. Both queued and running are in GOING_RUN_STATUSES, so the label does not change which runs are treated as going. The only cited consequence that could change behavior is an idle or waiting value on a non-latest active run. Nothing in the inspected code produces that combination for a queued successor.
The fake also cannot yield queued at the thread level. Its status is running when an active run exists, so the test-coverage remark is accurate but does not establish a defect.
Run updates now follow the active run before newer queued runs and post their replies in order.
Runs are still detected when they fall outside the recent-runs window, including silent active runs and queued runs.
Threads without an active run no longer trigger unnecessary waiting or reply posts.
The startup relay now uses active and recent run metadata to discover unaccounted runs and preserve their intended order. The thread-read response and test support provide this metadata, and new tests cover runs outside the recent window and delivered-mark states.

Expose run metadata extensions/t3code-remote/src/t3-client.ts, extensions/t3code-remote/src/t3.test-support.ts, extensions/t3code-remote/src/run-bridge.ts
Thread reads now include the active run and recent run IDs, ordinals, and statuses. Test support returns this metadata and handles 1 ms status polls. readActivity passes recent runs through.
Discover and order unaccounted runs extensions/t3code-remote/src/run-bridge.ts, extensions/t3code-remote/src/run-bridge.test.ts
The relay combines active and recent runs, polls candidates when needed, and returns unaccounted runs in computed order. Tests cover empty threads, active-before-queued ordering, runs outside the recent window, and delivered-mark combinations.
