Interim policy after two false-positive issues (everest-forms-pro #1243/#1244, closed).
Caveat: self-reported confidence is uncalibrated (94 then 60 for the same wrong claim), so this does not stop a confidently wrong diagnosis. Prompt-only change; 28 tests pass.
