Summary

• Initialize the latest AI-SDLC v3 scaffold for this repository.
• Add .ai/, .memory/, ADR, specification, traceability, eval, observability, MCP/A2A, and AI failure-mode review surfaces.
• Link AGENTS.md and README.md to the human and machine workflow manifests.
