Implements W3 on top of W2: atomic issue/revision/outbox transactions, insert-only revisions, fault and concurrency safety, exact PDF/XLSX parity, immutable persisted artifacts, and tenant-bound revocable capabilities.
Verification: cumulative pytest, ruff, configured mypy, artifact/layout/office, migration, and fault matrices green.
