Update the README for the full-stack features: --backend (hono/elysia), --orm (drizzle/prisma), --db (sqlite/postgres/mysql/d1), --layout, and the version/advanced flags (--channel/--pin/--install/--git/--node-engine). Adds a "Backend & full-stack (monorepo)" section (apps/web + apps/api + packages/domain, docker-compose for server DBs) and refreshes the project structure to include the new template and pkg files.
