Refs #4 #6 #9

macOS/APFS and SMB keep names in whatever Unicode form they were
written (often NFD), while Resolve, a user list or the DIT's MHL may
carry NFC. A clip listed in the other form was reported as "not in the
source MHL" and scan() returned two items for one file (D17, H10).

Preparation now resolves every path to the real name of the folder
listing (FS.real_name / FS.real_dir, one NFC map per folder, built only
when needed; ASCII paths list nothing extra), dedupes by that real path
and matches sequence patterns on NFC forms. The worker retries a missing
path against the DIT's ASC MHL paths (media_hashes of the card history)
and legacy .mhl keys by NFC form; the new generation keeps the on-disk
destination path. Tests cover both directions, scan dedupe, sequences
and legacy, and finish with ascmhl-debug verify.
