- Add comprehensive .gitignore parsing functionality in lib/utils.js
- Support for common gitignore patterns including wildcards (*, ?)
- Handle directory-only patterns (trailing slash)
- Implement pattern matching with regex conversion for complex patterns
- Cache gitignore patterns for improved performance
- Add shouldSkipFile function to check files against gitignore rules
- Update shouldSkipFolder to respect gitignore patterns
- Integrate gitignore checks into main TOC generation logic
- Add comprehensive test suite with 6 new gitignore-specific tests
- Update README documentation with gitignore functionality
- Maintain backward compatibility with existing exclusion methods
