ci: add Semgrep code-quality scanning

What this adds

• .github/workflows/semgrep.yml — runs Semgrep (p/default, p/security-audit, p/owasp-top-ten, p/javascript, p/typescript) on every push and PR so quality regressions get caught at PR time.
• .semgrepignore — skips test/fixture/vendored/generated paths so CI doesn't re-flag the noise the runner already filters out.

Semgrep is open source (MIT) and runs entirely in your CI. No data leaves the runner.
