v3.8.1: swarm lane keeps the live agent; no line outside /agt - #101

The lane cap kept the eight oldest cells, so past eight the newest - the working one - fell into '+n': a ninth fix attempt read as eight checks and +1. Every plain subagent of a session runs under 'adhoc', so a few Explore calls drew a session-long 'other ✓✓✓…' line. Same rule as the status line: only an /agt run gets one. Refs #100 Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Its silence clock was stamped only when a call started and a step ended, so one call past ten minutes (a full test suite) marked a live agent done. A call in flight now holds the timeout off, and its return counts as heard. Refs #100 Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
