The approved production staff correction failed with a generic message that concealed whether configuration, scope, identity-provider verification, or the database rejected it. Report a fixed execution phase and a sanitized failure code so recovery can identify the cause without logging credentials, identity records, provider response bodies, SQL, or arbitrary exception messages.
Unknown failures remain redacted. All authorization checks, provider operations and transaction behavior are unchanged.
Validation: 32 focused contract tests including secret-leak cases, contracts typecheck, lint, formatting, secret checks, and independent adversarial review.
