Add ProgramInfo module-ref launch path (tests only, rebased)

Summary:

• rebase the ProgramInfo module-ref launch work onto current main
• add ImpLab.Debugger.DAP.ProgramInfoLoader for loading ProgramInfo from module/decl
• allow DAP launch to accept programInfoRef with module and optional decl (fallback when programInfo is absent)
• keep existing JSON programInfo launch path unchanged

Validation:

• lake build

• this supersedes the previous PR branch that became stale during repo rename/rebase operations
