fix(ai): unattended agents: a cut-off answer is never a success, streamed tool calls parse or fail typed, failures carry their bill; continue, sampling, in-process tools - #767
Conversation
… an empty success
llm() and agent() validated the answer before they looked at the stop reason. A respond
call stopped at the output ceiling arrives with no arguments, both wires read that as {},
and {} satisfies an output of optional keys. An agent turn cut off while asking for tools
ran those tools on the same empty arguments. The stop reason is now read first.

…a typed, billed error — never the retryable code Both wire readers threw a transport error the moment a tool call's arguments did not parse, before the stop reason (Anthropic) or the usage (chat-completions) had arrived. A respond call cut off at the ceiling surfaced as X_AI_PROVIDER_UNAVAILABLE with the reservation released. - unreadable arguments are recorded, and the provider decides once usage is known: dropped on a truncated turn (X_LLM_TRUNCATED downstream), else X_LLM_TOOL_ARGUMENTS_INVALID carrying meta.billed, which the gateway records and does not retry - the chat-completions assembler starts a new call on a differing id at a shared index, and reads calls when the stream ends rather than at the finish reason - wire tests replay transcripts shaped like a captured OpenRouter run Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…meta.billed Every provider call of a run goes through a meter. Whatever ends the run after a call was started — truncation, refusal, schema-repair exhaustion, unparseable arguments, the turn ceiling, a tool loop, the deadline, the caller leaving — is thrown with meta.billed: the usage and cost of every call that answered, and a count of the calls that never reported usage, whose bill cannot be known and is never estimated. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…it with one user turn
agent() could only start a run, so correcting a finished answer re-ran every tool and re-paid
every turn. The action now carries .run() -> { output, transcript, turns, usage } and
.continue(input, { transcript, user }): the stored transcript is re-sent byte for byte (the
provider's prompt cache reads it back), one user turn is appended, and the loop resumes with
maxTurns and budget.tokensPerRun counted across the run and its continuations. The handle is
JSON the app stores, screened on the way in (X_AGENT_TRANSCRIPT_INVALID, 422).

…xact and rounded once
- llm() takes onCall: usage, cost and the running total per provider call
- onTurn / onCall carry repairOf on the turn after a rejected answer; X_LLM_OUTPUT_INVALID
lists the issues on meta
- cost is computed exactly in integers (Money at a finer scale, exactCostOf) and rounded up
once: per call instead of once per half, and per ledger when it is read instead of once per
call. 14 turns of 0.45 cents read 28 cents and now read 7
- a model price may state a scale: { minor: 75, currency: 'USD', scale: 3 } is $0.075 per million

…odel whose row takes them; forcedToolChoice: false
A verifier had no way to ask for a repeatable answer. Both declarations and GenerateRequest
take temperature and seed; a model row states sampling: { temperature, seed } to receive them
and they are left out for any other, since a reasoning model answers one with a 400.
A row may also say forcedToolChoice: false: one real call showed anthropic/claude-sonnet-5.5
rejecting the named tool_choice llm() sends for its single respond tool.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… a structured error
A tool failure is disclosed by the rule a problem document follows, so an app code with no
status row read as a 500 and its cause was withheld: the model could not see why its call was
refused. A ProjectableAction marked inProcess now shows an UltimateError thrown under a code
nobody declared a status for as { error: { code, cause, fix } }. An uncoded throw, a framework
5xx, a code the app registered >= 500 and a look-alike stay withheld; discloseErrors: false
opts out; a tool projected from an exposed action is unchanged.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…d is never an MCP tool
agent() refused any tool that was not mcp: { expose: true }, so a tool only the agent may
call had to be an action that claims to be exposable, and was then served to every external
MCP client. inProcessTool({ name, description, input, execute }) is a tool that is this
process's alone: its input is parsed by its own schema, it runs as ctx.actor under the
agent's policy, and no MCP catalogue, route, openapi.json or manifest row ever names it.
An action used as a tool is unchanged and must still be exposed.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…tion needs, or refuses Pointed at a slice an author wrote, the action generator behaved like a resource scaffold: it wrote entity/repo/policy tests for modules it had not written, merged the entity's admin labels into the catalogs, and emitted an action importing can<Slice>Write and <slice>Tag from a policy.ts that declared neither — a file that did not load, on which the generator then crashed with everything already on disk. A module already on disk gets no companion; a repo.ts without byId gets the lookup-free body; and a policy.ts lacking the imported exports is refused before anything is written (X_SLICE_POLICY_INCOMPLETE). The api/index.ts registration is unchanged: it is what names an action. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…re handed in as a rejection The gate's errors step reads test fix lines and test throws: tool-errors.test.ts cited 'x db console', which no build ships, and call-observability.test.ts threw a bare TypeError where it meant to hand the code under test a failing observer. It is now an HMAC-signed value (the framework's signing secret, its own label) bound to the agent, the model, the declared tool names and the actor's orgId; every tool call in it must name a declared tool; production on the development key refuses .continue(). A handle was only written on success, so failed continuations never counted against tokensPerRun. A failed .run()/.continue() now seals an updated handle onto the error (transcriptOf(error)) whose turns and usage include what the call was billed. Do not prescribe a lower token ceiling. If a declaration already sets maxTokens above 8192, this fix tells the caller to reduce it after a completed turn produced invalid JSON. Pass the current ceiling into this error and suggest a value above it, or omit the fixed number. As per path instructions, “Errors are instructions — every throw carries a stable X_* code, a cause, and an exact fix command.”

Review comment at @packages/ai/src/answer-errors.ts at line 57:
Update the fix guidance in the answer error so it does not recommend a fixed
maxTokens value of 8192 that could be below the caller’s current ceiling; pass
the current ceiling and suggest a higher value, or omit the numeric
recommendation.

Keep second-level re-exports marked as unknown. If policy.ts exports ./rules and rules exports ./more, inlineReexports removes the second star export. The policy check then treats the required names as missing and refuses a valid x g action or x g mutator. Preserve an unresolved marker when the one-level scan reaches another re-export, so the check does not claim those names are absent.

Review comment at @packages/cli/src/generate-reexports.ts at line 31:
Update the lookup passed to inlineReexports so re-exports encountered beyond the
one-level scan remain marked as unresolved rather than being removed. Preserve
the existing behavior for specifiers found in found, allowing policy checks for
x g actions and mutators to treat names behind second-level re-exports as
unknown.

…y grants, or assert nothing that depends on it
The generated unit test granted <feature>:write and expected the handler to answer, so a
handwritten canBlogWrite = can('blog:publish') shipped a failing test. The grant is now read
off a single literal can('<perm>'); when it cannot be read the handler assertions are left out
with a comment. The contract test makes no foreign-org claim against a policy it did not write.
A fresh slice's output is pinned byte for byte.

The pinned generator output carries an X_* literal, and a non-test module holding it read to the error registry scan as shipped source. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Found by an app running an unattended article-writing agent on 27.9.0. Each fix has a test that failed first.
