fix: #206 follow-ups: redact-then-cut attempt log, Codex quota codes, MiniMax salvage, slash passwords, linear core scrub; v2.28.1 - #207
Conversation
item 1) logAttemptFailure cut the upstream message to LOG_REASON_MAX_CHARS and left redaction to the logger, so a credential straddling the cut could fall below its pattern's minimum length and survive as a fragment. The HTTP attempt path redacts the message earlier; the Agent-SDK path does not. Both log lines now share one redact-then-cut helper. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…#206 item 4) postgres://u:p/ss@h was not matched: the password class stopped at '/'. A second branch allows '/' but is bounded to 256 characters, so each scan from '://' stays bounded and redaction stays linear (new timing cases), and it stops at '"' so it cannot run across a JSON field into an unrelated '@'. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…sswords (#206 item 6) scrubCredentials carried the quadratic shapes #205 removed from the server redactor: the userinfo pattern matched the scheme, and the JWT header was unbounded. 32 KB of 'a-' and the JWT-like runs took 12.7 s together. The scheme now stays outside the match, the JWT header is bounded with a lookbehind-anchored long-header pattern beside it, and a password holding an unencoded '/' is scrubbed through a 256-character bounded branch. Timing cases pin it. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… and over HTTP (#206 item 2) insufficient_quota (and the three sibling codes the Codex CLI maps to QuotaExceeded: credit_balance_exhausted, organization_spend_limit_exceeded, project_spend_limit_exceeded) classified codex:stream-error:other in a stream and rate-limited on a 429, so a spent account cooled down and was retried on a timer. Both now read credits-exhausted. Provenance: openai/codex codex-rs/codex-api/src/sse/responses_error.rs at f82e3659; the test frame is the shape the CLI's own test uses. Production logs were not searched for a captured frame. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
#206 item 3) A MiniMax body over the observation cap went through the salvage scanner, which kept no base_resp, so the driver hook saw no envelope and an in-body failure (1008, a spent balance) read as no failure at all, not merely as a generic one. base_resp is now an optional root slot beside error and code. …nt, Codex rule scope 1. The new slash-password branch ran through '?query' to an '@' inside a query-string secret, ate the 'password=' key and left the tail for the log (main redacted it fully). The branch now stops at '?', '#' and ':' (the colon also caps each scan, fixing a 50x slowdown on '://' runs). Regression cases in both suites; '://' and '://:' runs added to both cost suites. 2. Redact-then-cut could still emit a fragment: a long credential collapsing to [REDACTED] pulled text cut at the window's end into the kept range. When the window cut the text, its last 4096 characters are now never kept. 3. The Codex quota HTTP rules apply only on a 429, as the CLI reads them; the provenance comment no longer claims 'never retried' proves permanence. 4. 07-security documents the over-redaction price. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…e slash branch's edges 1. base_resp was an optional whole-object salvage slot: a long status_msg pushed it past its share, it was dropped, and the MiniMax failure settled as a success. Only base_resp.status_code is now kept, as a required slot; the free-text message never is. Test: a 400-character status_msg at a 1 KB cap. 2. Tests pin that the slash branch stops at a quote and the documented port-plus-'@' price; the Codex 429 rule is table-tested over every quota code. 3. Stream-errors header comment no longer says the HTTP rules are untouched; CHANGELOG names the exact timing inputs and the core JWT price. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

…ring key The slash-password branch's username still crossed '?', '&' and '=', and its password crossed '&' and '=', so 'postgres://db?sslmode=require&password=Ab:c/d@ef9' and 'https://host:8080/v1&api_key=ab@cdSECRET' lost their key and logged the tail; main redacted both. The branch now excludes ? # & = " from username and password, and the second alternative is main's pattern byte for byte, so the new branch only ever adds matches. Both repros pinned in both suites. Also: the Codex 429 type rule reads only insufficient_quota as a type, as the CLI does; docs say base_resp.status_code and state the branch's real stop set. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… CLI reads it Corrects a test this PR added. It asserted that a stream frame with no code and a nested type of insufficient_quota parks the account as credits-exhausted. The Codex CLI's stream parser (responses_error.rs at f82e3659) reads only the code and retries a type-only frame, so parking the account went beyond the cited source on no production evidence (#207 round-2 adversarial review). The test now asserts server-error / codex:stream-error:other, and the quota codes leave the in-stream type list. The HTTP 429 type rule (insufficient_quota) is unchanged. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…efix, salvage edges
1. The narrowed slash branch excluded '=', so a base64 slash password with '='
padding leaked whole; '={0,2}' just before the '@' is allowed again. Pinned in
both suites; the three query-string repros stay redacted.
2. Redact-then-cut could leave only an ellipsis when an early token collapsed;
the window now grows by what redaction removed (four rounds at most), so the
words before the token are logged up to the bound.
3. base_resp.status_code carries its own 32-byte ceiling: dropped past it,
never failing a Codex or Anthropic salvage.
4. Pre-existing in the same scanner: a root key named constructor/toString
holding an object threw a TypeError; slot lookup now reads own keys only.

…-error quota
1. Boot-path scrub: the '={0,2}' padding let the slash branch cover a key=
before an '@' ('jdbc:postgresql://h:5432/db;password=@s'), and core's key=value
pattern anchors on \b, not ? or &, so the secret leaked. Padding is now
refused right after one of that pattern's key names.
2. Boot-path scrub: dropping the scheme from the match (29d6cc6) let userinfo
fire with no real scheme before '://' ('x_y://a:password=@s'), which main left
to key=value. A linear lookbehind requires main's scheme again.
3. A quota code counts only on response.failed: the Codex CLI ignores a bare
error event, so one carrying a quota code no longer parks the account.
4. Docs 03/05, salvage comment and CHANGELOG match the code.

…s_code at budget 1. The quote window doubles up to 64 KiB (was four fixed rounds), so a 30 KB token or 400 collapsed keys still leave the words before them. 2. A cut never splits [REDACTED], and the quote is re-redacted and pulled back until it fits, so the logger's pass cannot push it past the bound. 3. base_resp.status_code gives way when the budget is full and is evicted before a required member would fail: it never fails a salvage main completed. 4. CHANGELOG names the CLI's fifth 429 code and why it stays rate-limited. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

Review summary — 25 file(s), 7 finding(s).
Critical 0 · Major 0 · Minor 7 · Nit 0
credential-redaction
Both scrubbers gain the slash-password userinfo branch with identical shapes; additions are redaction-additive and the documented over-redaction prices check out. Only gap: core's tests never pin the quote-stop and port-price outcomes its server twin pins for the same new pattern.
openai-oauth-stream-errors
Adds the four Codex quota codes as credits-exhausted in-stream and on HTTP 429, with tests pinning both paths, the non-429 escape, and doc rows that match the code. One stale neighbor: 03-providers.md:347 still says only a deactivated plan goes credits-exhausted.
release-version-chore
Bumps the whole tree to 2.25.9 consistently (five manifests, version.ts, Dockerfile label) with a CHANGELOG section for the six fixes. Three changelog defects: the [2.25.9] footer link is missing, its stream-matching claim contradicts the PR description, and one changed test file is unlisted.
concern-tests
Every changed behaviour ships a failing-on-revert, failure-case-first test; two small gaps: the "absent message" case that attempt-log.test.ts names is never exercised, and the stream-failure line's margin fix is pinned only through logAttemptFailure.
concern-api-contract
Contract verdict: no break — new exports (CODEX_QUOTA_CODES, scrubbedQuote usage) and classifications are additive, no signature changed, and 2.25.9 agrees across all seven version surfaces; one spec-table row over-claims the type axis for three quota codes the driver matches only as code.
concern-style-nits
Style is tight — the parallel scrub comments moved together and the CHANGELOG's hard claims verify. Two residues: docs/03's new openai-oauth row overstates the type-matched quota codes, and the slash-password branch plus its rationale is duplicated across the two scrubbers.
attempt-log-response-salvage (handoff)
Redact-then-cut log helper and MiniMax salvage check out: scrubbedQuote routes through logging/redact before the cut, and base_resp.status_code survives the cap fail-closed. One gap: doc 08's closed errorClass set omits the new openai-oauth:insufficient_quota signal.
No findings from: attempt-log-response-salvage, concern-security, openai-oauth-stream-errors (handoff).

Why this review is here
This repository defers code review to CodeRabbit, and developerz.ai reviewed this pull request only because CodeRabbit did not post a review for this commit. Once it reviews again, this bot stands back down.
Findings withheld — could not be cited
1 finding was withheld from this review and is NOT shown below — they could not be tied to code in this repository. It was not judged low-signal; it could not be verified, which is a different thing.
Every finding must cite real code before it is posted. Grounding needs a CodeGraph index on the server running the review; until there is one, findings that cannot be cited are withheld rather than shown.

Every `RouterError` message is router-authored and is already the client-facing body, so the line learns nothing the caller was not told — it just becomes findable. The line also carries what the request said about its body, by allowlist — `content-type`, `content-encoding`, `content-length`, `user-agent`, beside the session and client headers every completed line has — so an empty body, a compressed one, and the wrong content type are told apart without a packet capture. minor · docs This PR adds the HTTP-rule signal openai-oauth:insufficient_quota (openai-oauth.ts) to errorClass's closed set, but the doc's errorClass contract row (line 38) still enumerates only in-stream hook signals (codex:stream-error:<code>, minimax:base_resp-<code>), so the closed set the doc claims is incomplete once this lands.

Line 38 at head still names only the in-stream hook signals (codex:stream-error:<code>, minimax:base_resp-<code>) as what carries a signal into errorClass; the HTTP-rule signal openai-oauth:insufficient_quota from openai-oauth.ts's 429 typeRule (still present at head) fills the same closed set but is unenumerated.

Re-checked at 6daea7858a58: still open at docs/idea/08-observability.md:38. No reply declined the finding, and line 38 at head is unchanged: it still names only the in-stream driver-hook signals (Codex's codex:stream-error:<code>, MiniMax's minimax:base_resp-<code>) as what carries a signal into errorClass, while the HTTP-rule signal openai-oauth:insufficient_quota (openai-oauth.ts:255 typeRule at head) fills the same column unenumerated. The only edit to this file since the finding was the unrelated redact-then-cut row (~635).

minor · test The core suite pins the new slash-password branch but never pins the " stop or the port-plus-@ price that the server suite pins for the byte-identical branch (redact.test.ts:506-510), so a future edit to this copy can silently diverge and re-leak what the log redactor hides. Add both assertions here.

Re-checked at d2ee4a7d53b1: still open at packages/core/test/unit/scrub-credentials.test.ts:45. The suite still lacks both pins the finding asks for: no case exercises the " stop (a slash-URL beside a JSON field like {"url":"https://h:8080/a","owner":"x@y"}), and nothing asserts the documented port-plus-@ price (https://host:8080/a@b → https://[REDACTED]@b). The new tests at head (57–87) cover scheme lookbehind, padding and query strings only; the slash-branch test at 45–49 is unchanged, so the divergence risk stands.

…ct quota frame, salvage order 1. The doubling window could expose the head of a credential over 4 KB that straddled its end. The quote now redacts the whole message up to 64 KiB (the upstream error-body cap), so each credential meets its own terminator; past that, the 64 KiB scan drops its last 4,096 characters as before. 2. A quota code counts only from response.error.code of a frame whose own JSON type is response.failed: a root code or error, or a frame typed only by its SSE event name, is retried or skipped by the Codex CLI and no longer parks. 3. base_resp.status_code drops itself at a full budget before evictOptional, so it never evicts the error/code evidence main kept. 4. Salvage comments agree; 08-observability describes the quote scrub. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… HTTP envelope note 1. A full budget now evicts the ceiling slot (base_resp.status_code) first and the optional error/code only if still full, so a 64 KiB payload carrying both keeps the code main kept. Pinned at the exact budget edge. 2. The 429 quota rules are tested over both envelopes; the code comment and CHANGELOG say the CLI reads only 'error' and the driver also reads 'detail'. 3. The reviewer's three over-4 KB credential cases (JWT payload, long JWT header, userinfo password) pinned against the whole-message scrub (44a7760). 4. CHANGELOG lists response-salvage.test.ts. | | `4xx` other than `429` | **Do not retry.** A bad request is bad at every account; returning the upstream's error is the honest answer.
| | The upstream refuses the **model name** (`model-unsupported`, driver-classified: the `claude` CLI's "There's an issue with the selected model … may not exist or you may not have access to it", Codex's "`<model>` model is not supported when using Codex with a ChatGPT account") | **Retry the next candidate**, before any byte reaches the client. **No breaker strike, no cooldown, no recovery probe** — the account answered correctly about a name it never served, and the next account may serve that vendor. Recorded as `client_error`. When every candidate refused it, the client hears the first refusal itself: Codex's own `400`, or for a subscription a router-authored `404` "the Claude subscription does not serve the requested model". Routing's [model family](#candidate-filtering) keeps most of these from being attempted at all; this row is for what the family admits and the upstream still refuses. Until 2026-10-04 the first read `unknown` (a `502` that struck the breaker) and the second `invalid-request` (no failover). | | ||
| | **In-stream refusal on a 200 stream** (Codex `response.failed` or bare `error` event; classified by the `openai-oauth` driver hook, not the HTTP `rules` array) | Codes `invalid_prompt`, `invalid_request_error`, `context_length_exceeded`, `unsupported_parameter`, `unsupported_value`, `invalid_json_schema` → `invalid-request` (failover reads `client-error`; the breaker is **not** struck — a request-shaped refusal is bad at every account, but the account is not unwell). Codes `usage_limit_reached`, `rate_limit_exceeded` → `rate-limited`; the observer forces `rateLimit: null` so the verdict carries no estimated reset, and the breaker trips on the **first** hit (a stream-only rate limit is a fact about the account, not about the request). Code `account_deactivated` → `credits-exhausted`; the account moves to `exhausted` on the first hit (no clock, human action only). Any other code, or no code, → `server-error` (fail-closed; three in a row cool the account). The verdict's `errorClass` is the closed `codex:stream-error:<code>` signal (or `codex:stream-error:other`); the usage row carries the signal, never the upstream message. The code decides when there is one (top level, then the nested error object's `code`): a listed code maps as above, any other code is `server-error`. Only when there is no code is the nested error object's `type` read, and only for `usage_limit_reached` / `rate_limit_exceeded` / `account_deactivated` (the same lists the HTTP rules use, which read `type` even beside a code; in-stream, a code always decides): `invalid_request_error` is OpenAI's catch-all *type*, carried by account-level refusals such as `invalid_api_key` too, so a type never marks a refusal request-shaped. The flat event's own `type` is the event name `error` and is never read as a code. A frame too large for the observation cap is salvaged with its `error` and `code` kept, so a large echoed prompt does not hide the code; an error object too large to keep is dropped and the failure classifies `server-error`, never success. | | ||

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Describe MiniMax status-code salvage as conditional.
This row says an oversized frame keeps base_resp.status_code and therefore still identifies the MiniMax failure. response-salvage.ts can drop that slot when it exceeds 32 bytes or the shared budget is full. State that salvage retains the code only when it fits; otherwise classification may lack the MiniMax failure evidence.

Critical 0 · Major 0 · Minor 5 · Nit 0
failed-stream-salvage-and-redaction
Redact-then-cut logging, Codex quota codes, MiniMax salvage and slash-password scrubbing match their tests. Three minors: a provider-doc row over-narrows account_deactivated, the salvage's own comments contradict its give-way eviction, and core's padding guard leaves one slash-password shape unreda…
release-version-bump
Release plumbing is consistent: 2.25.9 agrees across the five manifests, version.ts and the Dockerfile label, and the CHANGELOG's claims match the sibling files this PR ships.
concern-security
No security boundary widens: the redactor and boot-scrub changes only add coverage with their documented prices, the salvage own-key fix closes a prototype-key throw, and I found one stale comment contradicting the new eviction rule but no reachable leak, injection, or auth defect.
concern-tests
Tests pin every changed behaviour with revert-failing cases; one gap: the 429 quota-type rule's insufficient_quota-only restriction has no negative test, so widening it to all four codes would pass the suite while parking accounts on type-only frames.
concern-api-contract
No contract breaks: every changed exported surface keeps its signature, the new errorClass/signal values stay inside the documented closed shapes, and all five manifests, version.ts and the Dockerfile label agree at 2.25.9 — one same-diff comment pair contradicts itself about whether the new salvag…
concern-style-nits
Style verdict: three texts this diff adds misstate the code — a salvage comment promising "never evicted" for a slot the scanner drops, a providers row scoping account_deactivated to 429, and a garbled scheme comment; no naming, dead-code, or duplication problems.
release-version-bump (handoff)
Release plumbing agrees: 2.25.9 across all five manifests, version.ts and the Dockerfile label, and the 2.25.9 CHANGELOG stream-hook wording matches the code's code-only matching. One docs defect: the [2.25.9] heading still has no footer link definition.

… KiB, error evidence outranks base_resp 1. Past 64 KiB the quote's scan ends at the last whitespace before the mark; no credential pattern spans whitespace, so none can straddle the cut. A whitespace-free message keeps the margin fallback. Pinned at 200 and 2000. 2. An optional error capture meeting a full budget releases a held base_resp.status_code before giving way (the other order was fixed in round 4). 3. Wording: CHANGELOG, docs 03/08, the stream hook header (with the one deliberate widening over the CLI's typed parse), and a wrapped comment. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
1. Tests at exactly 65,536 and 65,537 characters pin both quote paths, and a whitespace-free message over 64 KiB pins the fallback. That test showed the 4,096-character margin still leaks a longer straddling credential, so the margin is gone: a message with no whitespace in its first 64 KiB is one token nothing can safely be cut from, and only '…' is logged (main: '[REDACTED]…'). 2. Test name no longer overclaims; the stream gate comment names the CLI's stricter typed parse as the one deliberate difference. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…rror.code decides quota 1. Error evidence at a full budget released base_resp.status_code before knowing it would fit; when it dropped anyway both were lost and a MiniMax 1008 on a 200 settled as success. The ceiling slot is now lent and given back if the error capture drops. 2. A root code or error object no longer hides a quota code at response.error.code of a response.failed frame, which is where the Codex CLI reads it. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…arker pull-back and the stream line Every change the reviewer showed survived the suite is now caught (checked by mutation on a scratch backup): the whole-message threshold off by one, the whitespace scan start, the '…' output, the marker pull-back in fitted(), and reverting the stream line to 2.25.8's window. The straddle cases run through both log lines. Wording: a credential's secret part, not its pattern, never spans whitespace; the 429 rule comment names the mixed limit-and-quota difference. credential-redaction
Slash-password redaction lands on both scrubbers with docs, cost bounds and tests; I traced the new patterns case by case and they hold. Residuals: the two implementations still diverge on DSN key=value secrets (server misses what core scrubs), and core's padding guard can leak a crafted base64 tai…
openai-oauth-stream-errors
Adds Codex quota-code exhaustion classification (stream hook gated to response.error.code on response.failed; HTTP 429 rules) with CLI provenance, tests, and docs.
dataplane-usage-attempt-logging
Redact-then-cut logging, the base_resp salvage slot and the prototype-key guard are sound; four minors: the stream-failure line's new scrub path is untested, the errorClass doc row omits the new openai-oauth quota signal, and two new-test comments describe the margin this diff deleted.
concern-tests
Changed behaviours carry revert-sensitive failure-case tests, but four guards are missing: logStreamFailure is never driven, one test skips its named absent path, the quota-code set can drift test-free, and one new salvage branch is unpinned.
concern-api-contract
No exported signature, schema, route, or default change breaks a caller; one rewritten spec sentence misplaces the stream-hook quota-code restriction onto account_deactivated, which the code leaves unrestricted.
concern-style-nits
One misleading doc claim and two nits: the docs restrict stream account_deactivated matching like the four quota codes though the code doesn't, a test comment still describes the removed 4,096 window, and quotaCode can hold a non-quota code.
credential-redaction (handoff)
No redaction pattern spans whitespace, so the 64 KiB cut invariant holds; the new slash-password and padding tests trace correct. Two minors: the server redactor still leaks the tail of ;password=@secret DSNs core now guards, and core lacks the quote-stop pin.
No findings from: release-version-metadata, concern-security.

The flat event's own `type` is the event name `error` and is never read as a code. openai-oauth-stream-errors
Adds four Codex quota codes to the openai-oauth driver: 429 HTTP rules and the stream hook (gated to response.error.code on a response.failed frame) classify credits-exhausted; every other shape stays server-error. Logic is fail-closed and well-tested; two minor doc/test gaps.
secret-redaction
Reworks URL-userinfo redaction in both scrubbers: a bounded slash-password branch joins the frozen original; core adds scheme and key-name lookbehind guards.
release-version-bump
Version 2.25.9 agrees across version.ts, all five manifests and the Dockerfile label; the CHANGELOG's named files and tests all exist in this PR. One defect: the [2.25.9] heading has no footer link definition (ends at [2.25.8]:), so it renders as literal text.
concern-tests
Test surface is strong — the 64 KiB boundary, negative-direction and revert-discriminating cases are real — but three minor gaps: the absent-message path the attempt-log test names, two branch pins missing from the core scrub suite, and the documented limit+quota precedence left unpinned.
concern-api-contract
No signature, schema, or stored-shape contract breaks; one spec drift: the in-stream doc pins account_deactivated to a location restriction the hook never enforces (minor).
concern-style-nits
No style debt beyond two stale-prose spots: a garbled scheme-lookbehind comment (echoed in the CHANGELOG) and a regression test still documenting the deleted 4,096-character redaction margin.
usage-salvage-attempt-logging (handoff)
Redact-then-cut logging and the base_resp salvage slot match their contracts, with real boundary tests. Open: the errorClass row omits the new openai-oauth:insufficient_quota signal, the 64 KiB boundary prose contradicts its inclusive scan, and the absent-message branch ships unasserted.
openai-oauth-stream-errors (handoff)
Quota-code classification wiring matches its stated contract in both the stream hook and the 429 HTTP rules. Defects: two doc rows mis-scope account_deactivated or contradict the new quota-location suppression, and the deliberately excluded fifth quota code has no pinning test.
secret-redaction (handoff)
Redactor and scrubber changes check out: new userinfo/JWT patterns match their comments, lose no match versus main, and tests pin the leaks, prices, and linear cost. One gap: 07-security.md still does not teach the redact-then-cut quote window this PR's own description records on #208.
No findings from: usage-salvage-attempt-logging, concern-security.

minor · test The deliberate exclusion of organization_usage_limit_exceeded — the one thing keeping a 429 with that code at rate-limited instead of exhausted — is pinned only by this comment; no test asserts it. Add a case classifying that code on a 429 as rate-limited so re-adding it to CODEX_QUOTA_CODES cannot silently park usage windows.

Re-checked at a11a5f1e8062: still open at apps/api/src/providers/drivers/openai-oauth.ts:257. The exclusion is still pinned only by the comment at head (lines 257-258); the changes since the finding add only comment rewording here, and the only new driver-adjacent test is for the stream hook's quota-beats-root-code path (openai-oauth-stream-errors.test.ts), not a 429 test asserting organization_usage_limit_exceeded classifies rate-limited.

Re-checked at 008f22459155: still open at apps/api/src/providers/drivers/openai-oauth.ts:257. The deliberate exclusion of organization_usage_limit_exceeded from the 429 quota rules is still pinned only by the comment at lines 257-258. The changes since the finding reword that comment and add one driver-adjacent test — the stream-hook case (openai-oauth-stream-errors.test.ts:183, quota code beats root code) — but no test classifies that code on a 429 as rate-limited, so re-adding it to CODEX_QUOTA_CODES would still not fail anything.

Re-checked at 478f5964817c: still open at apps/api/src/providers/drivers/openai-oauth.ts:257. At head the 429 quota rule (line 257) still classifies via QUOTA_CODES without organization_usage_limit_exceeded, keeping that code rate-limited only by design — the new 429 tests (openai-oauth.test.ts:265-270) cover credit_balance_exhausted as a type and usage_limit_reached beside insufficient_quota, but no test asserts organization_usage_limit_exceeded on a 429 is rate-limited, so re-adding it to CODEX_QUOTA_CODES would still pass CI.

Re-checked at 7cf0db4dbd90: still open at apps/api/src/providers/drivers/openai-oauth.ts:257. Still open: the 429 quota rule (line 257) still runs on QUOTA_CODES, which omits organization_usage_limit_exceeded by prose only (the rationale moved to the CODEX_QUOTA_CODES doc comment). The new 429 tests (openai-oauth.test.ts:265-270) cover credit_balance_exhausted as a type and usage_limit_reached beside insufficient_quota; none classifies organization_usage_limit_exceeded on a 429 as rate-limited, so re-adding it to CODEX_QUOTA_CODES would still pass every test.

At head the 429 quota rule still runs on QUOTA_CODES (line 257), which omits organization_usage_limit_exceeded by prose only — the rationale now lives in the CODEX_QUOTA_CODES doc comment, not a test. The new 429 tests (openai-oauth.test.ts:265-270) cover a credit_balance_exhausted type and usage_limit_reached beside a quota type, but no case classifies organization_usage_limit_exceeded on a 429 as rate-limited, so re-adding it to CODEX_QUOTA_CODES would still pass CI.

The new 429 test.each (lines 265-270) still covers only credit_balance_exhausted and usage_limit_reached; no case classifies organization_usage_limit_exceeded on a 429 as rate-limited, so re-adding it to CODEX_QUOTA_CODES would still pass CI.

Re-checked at 6daea7858a58: still open at apps/api/test/unit/providers/openai-oauth.test.ts:265. At head the 429 tests still only cover quotacodes-generically (line 243 loops CODEX_QUOTA_CODES, so re-adding the code would still assert credits-exhausted and pass) and the test.each at 265-270 covers only credit_balance_exhausted (type) and usage_limit_reached+insufficient_quota. No case pins organization_usage_limit_exceeded on a 429 as rate-limited; the source rule (openai-oauth.ts:257) omits it by prose alone. No reply declines the finding.

| | `request failed` (`transport`) | `errorClass`, `errorCode`, `status`, and **`error`**: the thrown `RouterError`'s whole `cause` chain, innermost first, scrubbed and bounded by the logger's `Error` rendering. Every `RouterError` message is router-authored and is already the client-facing body, so the line learns nothing the caller was not told — it just becomes findable. A `translation_failed` line names the field that was refused. The line also carries what the request said about its body, by allowlist — `content-type`, `content-encoding`, `content-length`, `user-agent`, beside the session and client headers every completed line has — so an empty body, a compressed one, and the wrong content type are told apart without a packet capture. Values are bounded to 200 characters; no credential-bearing header is on the list. | | ||
| | `upstream attempt failed` (`transport`) | `accountId`, `attempt`, `status`, `failureKind`, plus **`signal`** (which classifier rule decided — `http-status:429`, `claude-sdk:unclassified`), **`reason`** (the router-authored sentence), and **`upstreamMessage`** (the provider's or the SDK's own words, bounded to `LOG_REASON_MAX_CHARS` and scrubbed). | | ||

Up to 64 KiB the whole message is scrubbed; past it, the scan ends at the last whitespace before 64 KiB, so no credential's secret part straddles the cut (a message with no whitespace in its first 64 KiB logs only `…`). A cut never splits `[REDACTED]`, and the quote fits the bound after the logger's own pass).

minor · docs The scan is inclusive: lastWhitespaceBefore (attempt-log.ts:75) starts at index 65,536, so whitespace exactly at the 64 KiB mark keeps the head — this diff's own boundary test pins that — while the row's "last whitespace before 64 KiB" and "no whitespace in its first 64 KiB logs only …" claim … for that input. State the boundary as "at or before the 64 KiB mark".

…at and the absent quote; spec row 1. A status_code repeated inside one base_resp object is pinned (the round-7 test now repeats the parent, which the round-8 release handles, so the inner release line had no test; deleting it now fails). 2. An attempt with no upstream message logs no upstreamMessage field (returning '…' or '' now fails). 3. docs/idea/09-deployment.md: the observation-cap row lists the error/code evidence and base_resp.status_code, and the 32-byte lent exception to the bound. 4. CHANGELOG [2.25.9] link; the parent-release comment names ceiling slots only and the raw-byte key compare (#209). Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

openai-oauth-stream-errors
Adds Codex quota-code classification in-stream and on 429 so spent balances park as credits-exhausted (non-negotiable 7). The stream hook's quota-location precedence matches its documented CLI-provenance contract and is well tested; one documented HTTP precedence branch lacks a pinning test.
response-salvage
Salvage change verified: hand-traced the lend/giveBack accounting and all seven new numeric tests reproduce, including the ≤maximumBytes bound. One gap: no test lends again after a kept borrower clears lent, so a regression in that clear would pass CI.
attempt-log
Redact-then-cut scrubbing lands for both upstream failure log lines with a 64 KiB scan bound; the helper is leak-safe for redactor-matched credentials within this slice. One stale test comment still teaches the retired maxChars+4096 window; one invariant handed to the redact.ts reviewer.
release-metadata
2.25.9 plumbing checks out: five manifests, version.ts and the Dockerfile label agree, and the changelog matches the shipped files. Two doc defects: the [2.25.9] heading lacks its footer link, and the 03-providers openai-oauth row over-scopes its 429/type qualifier onto account_deactivated.
concern-tests
Four test gaps: the absent-message branch is named but never asserted, the core scrub suite lacks the quote-stop and port-@ pins its server twin has, and two documented Codex 429 edges — limit-beats-quota rule order and the organization_usage_limit_exceeded exclusion — are unpinned.
concern-api-contract
No exported signature, schema, signal shape, stored value or version pin breaks for existing callers; one rewritten spec row over-scopes the new in-stream location rule to account_deactivated, contradicting the hook's code.
concern-style-nits
No naming, structure or dead-code debt introduced; seven doc/comment defects: two rows misstate account_deactivated's matching axes, a wrong redaction-price example, a missing 2.25.9 footer link, an unasserted absent-message branch, and two stale or garbled comments.
release-metadata (handoff)
Versions agree at 2.25.9 across every manifest, version.ts and the Dockerfile label. Three doc defects: the 2.25.9 changelog heading lacks its footer link definition, 03-providers' openai-oauth row misstates account_deactivated's classification axes, and 08's errorClass enumeration omits the new HT…

Exception: a quota code at `response.error.code` of a frame whose own JSON `type` is `response.failed` decides even beside a root `code` or `error`, which the Codex CLI ignores on that frame. Only when there is no code is the nested error object's `type` read, and only for the limit and deactivated codes above, never a quota code, which the Codex CLI's stream parser reads only as a code (the same lists the HTTP rules use, which read `type` even beside a code; in-stream, a code always decides): `invalid_request_error` is OpenAI's catch-all *type*, carried by account-level refusals such as `invalid_api_key` too, so a type never marks a refusal request-shaped. The flat event's own `type` is the event name `error` and is never read as a code. A frame too large for the observation cap is salvaged with its `error` and `code` kept (and MiniMax's `base_resp.status_code`, so an oversized in-body MiniMax failure still reads as one), so a large echoed prompt does not hide the code; an error object too large to keep is dropped and the failure classifies `server-error`, never success.

Review summary — 25 file(s), 9 finding(s).
Critical 0 · Major 1 · Minor 8 · Nit 0
credential-redaction
Hardens credential scrubbing: slash-password userinfo in both redactors with padding and query-anchor guards, bounded JWT headers in core, shape tests included. One gap: core's new scheme lookbehind scans back unbounded on pure scheme-char runs — quadratic where the old code was linear.
openai-oauth-stream-drivers
Classifies the four Codex quota codes as credits-exhausted on a 429 and at the CLI's in-stream location; driver code, hook precedence, and tests are internally consistent. One finding: the new 03-providers row misstates account_deactivated's status and stream scope.
release-version-bump
Coordinated 2.25.9 bump: all five manifests, version.ts and the Dockerfile label agree, and the CHANGELOG entry names only files this PR actually touches. One defect: the new [2.25.9] heading lacks its footer link definition, so it renders unlinked where every release since 2.25.0 links its tag.
concern-tests
Every changed behaviour ships failing-first tests that pin the fix; three small gaps remain: the absent-message path the test names is unasserted, the core redactor copy lacks the quote-stop and port-price pins its server twin has, and the Codex quota rule's excluded siblings are unpinned.
concern-api-contract
No exported contract breaks: the driver, redactor and salvage changes are additive (new quota codes, wider redaction, one salvage slot) and all six version sites agree at 2.25.9. Findings: one stale CHANGELOG link and three still-unpinned behaviors/tests.
concern-style-nits
Style verdict: mostly clean — one over-claiming new comment in the salvage scanner and two provider-doc rows whose quota-code restrictions misdescribe account_deactivated, plus two nits (a garbled scheme sentence and a duplicated response-error read).
credential-redaction (handoff)
Credential-scrub hardening is sound: both userinfo branches exclude whitespace so the attempt-log cut invariant holds, docs 07 match the code, and cost tests pin linearity.
release-version-bump (handoff)
Coordinated 2.25.9 bump is correct: all five manifests, version.ts and the Dockerfile label agree, and the CHANGELOG entry matches the described changes. One defect: the new heading lacks its footer link definition, so it renders as literal text.
No findings from: usage-salvage-and-attempt-log, concern-security.

credential-redaction
Ports the slash-password and linear-scan forms into both redactors with tests; the server userinfo branch missed the padding guard its core copy got, so a DSN-style ;password=@secret still leaks its value into log lines — one major security finding, plus a missing core test pin.
openai-oauth-drivers
Driver logic is sound: the stream quota gate, 429-only HTTP rules, and non-CLI-shape suppression all match their tests and docs. One docs defect: 03-providers.md's new openai-oauth row folds account_deactivated under the 429/stream-location constraints that only bind the four quota codes.
release-version-bump
Version 2.25.9 agrees across the five manifests, version.ts and the Dockerfile label, and every CHANGELOG entry names files this PR really changes. One release-record gap: the Documentation bullet omits the docs/idea/09-deployment.md env-row update this diff also makes.
concern-tests
Every changed behaviour carries a revert-failing test except two: the 429 quota rule's exclusions and the core scheme-guard test, neither of which can fail on the regressions they name.
concern-api-contract
No signature, schema, route, config, or stored-shape contract changes; the API surface is additive and caller-safe. Two new doc claims in the signal table and the redaction price list contradict the code they describe.
concern-style-nits
Six #206 fixes land as described; style lane finds one misleading docs scope (account_deactivated welded into the quota codes' 429/stream-only restriction) and two nits: a 2.25.9 Documentation bullet omitting docs/idea/09, and attempt-log's now-unused bounded helper.
usage-salvage-attempt-log (handoff)
Both log lines share the new redact-then-cut helper and the salvage slot's ceiling, lend and last-wins mechanics check out against the tests. One doc gap: the errorClass contract row (docs/idea/08-observability.md:38) still omits this PR's openai-oauth:insufficient_quota signal.
credential-redaction (handoff)
Both redactors gain a bounded slash-password userinfo branch; core adds a scheme lookbehind and a long-JWT-header pattern, and the tests plus doc 07 match the code.
No findings from: usage-salvage-attempt-log, concern-security, release-version-bump (handoff), openai-oauth-drivers (handoff).

codex-oauth-stream-salvage
Codex quota codes now park accounts in-stream (only at the CLI's response.error.code location) and on 429s; MiniMax base_resp.status_code survives salvage with sound lend/evict accounting — both verified against their tests.
attempt-log-redaction
Redact-then-cut attempt logging, slash-password userinfo scrubbing, and the linear core scrub are sound, with tests pinning each #206 item. One parity gap: the server redactor's new = padding lacks core's key-name guard, so ;password=@… logs its secret behind a [REDACTED].
release-version-bump
Release chore to 2.25.9 over the six #206 fixes. All version surfaces agree (5 manifests, version.ts, Dockerfile label, changelog heading + link definition); changelog file/doc/test lists match the sibling set.
concern-security
Security verdict: containment only widens — both userinfo patterns add matches only, scrubbedQuote redacts before any cut with sound whitespace/marker bounds, salvage paths stay fail-closed with static slots, signals stay closed-set; one minor test-parity gap remains on the core scrub copy.
concern-tests
All six fixed behaviours ship tests that fail on revert (boundary, negative and malformed-shape cases included), but three documented decisions — the excluded organization_usage_limit_exceeded, the 429 limit-beats-quota rule order, and the core scrub's quote-stop/port price — are pinned by prose…
concern-api-contract
No exported signature, schema, route, config key, stored shape or version surface changes or breaks; the one contract defect is the new provider-spec row misstating the account_deactivated classification gates.
concern-style-nits
Style verdict: two rewritten doc sentences now contradict the driver on when account_deactivated parks an account as exhausted; the rest of the changed names, comments, and changelog claims match the shipped code.
attempt-log-redaction (handoff)
Redact-then-cut logging and the slash-password scrub rework match their tests and docs. Gaps: the API userinfo branch lacks core's padding key-name guard (a ;password=@ secret survives redactValue), 08's errorClass row omits openai-oauth:insufficient_quota, and the core scrub suite lacks the serv…

After a primitive, string or container value at base_resp.status_code ended, a
second value with no key between ('"status_code":0 1008', invalid JSON) re-entered
beginValue and was captured, so an oversized malformed MiniMax frame read as a
spent balance and parked the account, where main gives no signal. A ceiling slot
now captures only the first value after its key; a second one drops the slot as
malformed, as finish() treats any malformed ceiling slot.
Tests for the reviewer's surviving mutants, each now killed: the whitespace cut

The round-11 guard covered a second value under status_code only; a second or
keyless object under base_resp, or a missing colon, still supplied the code, so
an oversized invalid frame parked a MiniMax account where JSON.parse (and main)
see nothing. Instead of guarding shapes one by one, the scanner now checks

key or its colon belongs, blocks the ceiling slot for the rest of the frame and
releases any held or lent copy. Valid JSON never trips it (guards pin a primitive
parent, extra members and arrays). Mutants killed: no block, either half of the
condition, per-byte freshness, the capture gate, and keeping lent bytes.
Also pins the rest of the over-64 KiB cut set ('_', '=', '&', '?', newline-only)
and four more malformed non-ceiling members.

… files under 300 lines response-salvage.ts had grown to 354 lines and openai-oauth.ts to 303 against the house limit of 300 (flagged by the #207 round-12 adversarial review). The slot tables, byte constants, slotKeys and the pure rebuild step move unchanged to response-salvage-slots.ts; the byte-level state machine stays. The Codex 429 quota-rule rationale moves beside CODEX_QUOTA_CODES. No behaviour change. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…e cut; more second-value rows The stream line's redact-then-cut at the length bound had no test (logging it through bounded() passed the suite and logged a key head). The key-across-the- bound case now runs through both lines. Four more invalid second-value rows (0 :1008, 0,:1008, 0 "x", 1008,0) pin the structural block. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…stickiness and scope Per the operator's call (document, merge, release), this closes the cheap items both round-13 lenses found and states the scanner's limit plainly: 1. blockCeiling also drops a status_code capture still running, so its bytes stop counting (retainedBytes pinned exactly). 2. Exact-output tests: a violation after the code releases it; a violation under an earlier root key keeps the block in force; the block drops only the code, not other evidence. Each mutant now fails. The vacuous '"a":1 "base_resp"' case is replaced; three more malformed-member paths are pinned. 3. CHANGELOG and comments name the two modelled violations and say other malformed frames can still yield their code (accepted, #209). Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

openai-stream-and-usage-salvage
Codex quota-code classification and the MiniMax salvage slot match their tests; the defects are the new doc rows over-restricting account_deactivated, which the code classifies from any listed code position on any status.
concern-security
Security verdict: every redaction/scrub change only adds matches and holds under adversarial reading (input-anchored lookbehind can't be dodged by backtracking, whitespace-cut invariant is sound, own-keys slot lookup fixes the crafted-root-key TypeError); one residual, author-disclosed and #208-tra…
concern-tests
Every changed behaviour ships failing-on-revert tests plus edge and mutant pins; the earlier logStreamFailure and absent-message gaps are closed in this tree. One documented boundary, organization_usage_limit_exceeded staying rate-limited on a 429, is pinned only by a comment.
concern-api-contract
No contract breaks: exported signatures, driver-hook and redactor output shapes stay call-compatible, no schema/route/config/default changes, versions agree at 2.25.9; the new credits-exhausted classifications and signal values are deliberate, tested, documented in-PR.
concern-style-nits
Readability is sound — names and comments match behavior, no dead code I can prove — except one over-scoped doc claim: the credits-exhausted rows attach the 429/stream-location limits to account_deactivated too, which the classifier does not.
secret-scrubbing (handoff)
The scrub group checks out: redact-then-cut attempt log, bounded slash-password branch, and core's linear scrubber match their prose and pass their pins. Two gaps: 08's errorClass row omits the new openai-oauth:insufficient_quota signal; the core suite lacks the quote-stop/port-price pins.
openai-stream-and-usage-salvage (handoff)
Codex quota codes classify credits-exhausted in-stream (response.error.code of a response.failed frame only) and on a 429; MiniMax base_resp.status_code survives salvage under a 32-byte ceiling.

minor · test The driver comment leaves organization_usage_limit_exceeded out of the 429 quota rules, but no test pins it: the 429 suites map over CODEX_QUOTA_CODES, so adding the code to that list flips a 429 to credits-exhausted with nothing failing. Add a { error: { code: "organization_usage_limit_exceeded" } } entry here so the documented rate-limited verdict is machine-enforced.

openai-oauth-stream-errors
Codex quota codes now classify credits-exhausted in-stream (only from response.error.code of a typed response.failed frame) and on 429s only; precedence, suppression of other shapes, and HTTP rule gating all match the tests and comments. One test gap on the detail-envelope type path.
release-version-docs
Version bump to 2.25.9 is consistent: version.ts, all five manifests, the Dockerfile label, CHANGELOG heading and its link definition all agree. Two doc contradictions remain: the new 03/05 rows over-restrict account_deactivated's type matching, contradicting each other and the 05 cell's own text.
concern-tests
Changed behaviours all ship failing-on-revert tests and the suite is precise; the one gap is two salvage tests whose subset matchers cannot see the base_resp eviction they name.
concern-api-contract
No API surface, schema, stored shape, or version pin breaks: new exports/slots/signals are additive, the quota-code demotion only narrows toward main's behavior, and 2.25.9 is consistent across all seven version surfaces.
concern-style-nits
Comments and docs mostly track the code; one docs row over-scopes the new Codex quota restrictions to account_deactivated (minor), plus two nits: an orphaned cut-first log helper and an undocumented divergence between the twin userinfo scrubbers.
No findings from: usage-salvage-attempt-log, secret-redaction, concern-security.

credential-scrubbing-attempt-log
Redact-then-cut attempt logging and the bounded slash-password userinfo branch in both scrubbers check out against their tests; no credential leak found. Two smaller gaps: the errorClass doc row omits the new 429 quota signal, and the core suite lacks the quote-stop/port-price pins.
openai-oauth-stream-errors
Codex quota codes now classify credits-exhausted in-stream and on 429s; hook and tests agree. Three minors: two doc rows mis-scope the new restrictions over account_deactivated, which still parks from any status and code position, and the 429 detail.type path lacks a test pin.
concern-tests
Test surface is strong — every #206 fix ships a main-failing, precisely-asserted regression test (attempt-log drives the unredacted-SDK seam directly, salvage pins byte accounting and last-wins, redaction pins boundary and timing) — except the documented exclusion of the fifth Codex 429 quota code,…
concern-api-contract
No signature, schema, route, env or default contract changes: exports keep their shapes, the redaction and quota-code widenings are additive and documented, versions agree at 2.25.9 — the only issue is the new providers-table row misstating account_deactivated's classification contract.
concern-style-nits
Style and readability hold across the diff: names, comments and docs track the code, except one over-broad restriction claim in the new providers table row plus two folding-grade nits.
credential-scrubbing-attempt-log (handoff)
Redact-then-cut attempt-log quotes, Codex quota codes → credits-exhausted, MiniMax salvage slot, and slash-password scrubbing in both redactors; the scrubbing code and tests verify clean under line-by-line regex tracing.
openai-oauth-stream-errors (handoff)
Stream and 429 quota classification matches the corrected CHANGELOG claim exactly — narrow response.failed/response.error.code gate, 429-only HTTP rules over both envelopes, closed signals, organization_usage_limit_exceeded left rate-limited.

Both redactors gain the slash-password userinfo branch and the attempt log lines now redact before cutting; I traced every new branch, the padding guard, and the cut paths and found no leak or main-behavior regression.
openai-oauth-stream-errors
Codex quota classification matches its description: location-gated in-stream, 429-only over HTTP, with the precedence matrix pinned by tests. Two minor gaps: the quota mask drops a limit code arriving at the CLI-read location beside a root-error quota code, and one gate corner is untested.

Readability holds except two rewritten docs cells misattach the new 429/response.failed restriction to account_deactivated, which the hook does not restrict — actively misleading; plus one garbled comment duplicated in the CHANGELOG.

Coherent 2.25.9 bump: the version agrees across the five manifests, version.ts, the Dockerfile label and the CHANGELOG link. credential-redaction
Slash-password userinfo scrubbing lands in both redaction layers with bounded scans and leak-interplay tests; the new branches only add matches over main.
release-packaging
Version 2.25.9 agrees across all six manifests, version.ts and the Dockerfile label, and the new CHANGELOG tag link resolves. Two doc defects: the new 03-providers row mis-scopes the quota-type clause against this PR's own 05 row, and 08's errorClass set misses the new HTTP-rule signal.
concern-security
No redaction, auth or deserialization surface widens — the userinfo/attempt-log changes only add redaction and the salvage parser is key-set-bounded; one rewritten doc sentence now contradicts the stream hook's account_deactivated handling.
concern-tests
Every changed behaviour is pinned by tests that fail against the reverted code (regressions, salvage accounting, redaction edges, version agreement); the one gap is the deliberately excluded fifth Codex 429 quota code, which no test guards.
concern-api-contract
No exported signature, schema, env default or stored shape breaks in this diff; the one contract defect is a rewritten routing-doc sentence that mis-scopes the quota-code location restriction onto account_deactivated, which the stream hook classifies more broadly.
concern-style-nits
Style lane verdict: names, comments, removed-margin docs, and the five-manifest version bump all agree with the code; the one defect is a docs row that misstates how account_deactivated matches.
release-packaging (handoff)
Release chore is consistent: 2.25.9 agrees across all five manifests, version.ts, the Dockerfile label and the CHANGELOG footer. Two doc defects: docs/08's errorClass closed set omits the new openai-oauth:insufficient_quota signal; docs/03's stream-gate clause over-scopes to account_deactivated.

Both scrub layers gain slash-password userinfo redaction with bounded, linear patterns; branch-2 fallthrough preserves main's behavior exactly and the new tests pin the trade-offs. Core's suite still lacks the quote-stop and port-price pins its server twin has.
docs-idea
Two changed doc rows contradict their siblings: 03's new openai-oauth row strips account_deactivated's nested-type paths that 05:459 still documents, and 08:636 wasn't taught the 64 KiB margin its twin claims is shared. Rest of the doc updates match the shipped behavior.
concern-security
Security verdict: clean — redact-then-cut holds for every redactor pattern (secret charsets exclude whitespace, so the 64 KiB cut cannot split a match), the new userinfo branch only adds matches and provably skips query-string passwords, and the salvage scanner allowlists slot keys with no prototyp…
No auth or injection surface widens: every security-relevant change in my files tightens it — redact-then-cut log gates, slash-password scrub, 64 KiB cut bounds, a prototype-pollution guard and cost bounds are all pinned by tests, and the docs match the pinned behavior.
No auth or injection surface widens; one redaction shrink: a long-header JWT glued after - now passes core's boot scrubber raw where main's unbounded pattern caught it, contradicting the PR's differential-fuzz claim.
concern-tests
No changed behaviour in this slice yields a new, anchorable test gap: the code-level claims (redact-then-cut, 429/location-scoped quota codes, salvage ceilings, last-wins) are internally coherent and each names a test file that ships with this PR; the residual coverage gaps were already posted by t…
Tests bind tightly overall — every changed behaviour has a revert-failing test with exact assertions; two gaps: the in-stream quota-type exclusion is untested on response.failed frames, and the documented 256-character slash-password bound has no boundary test.
Test coverage pins every new core scrub branch, the lookbehind, the long-JWT path and the linear-cost bound with revert-revealing cases; one gap remains — the userinfo branch's quote-stop and innocent-JSON boundaries are unpinned in core while its byte-identical twin suite pins both, so the copies…
concern-api-contract
No contract breaks: exported signatures (redactValue, inspectCodexStreamFrame, logAttemptFailure/logStreamFailure, createResponseSalvage) are unchanged, the new quota signal and base_resp slot are additive, and the doc tables enumerating them go to their reviewers as handoffs.
No exported signature, enum, route or stored shape breaks (additions are additive and CI-pinned), but the two spec pages now disagree on which stream frames park an account_deactivated account and the new 256-character redaction ceiling ships without a pinning test.
No contract moves: scrubCredentials keeps its signature and only adds matches, 2.25.9 agrees across all held manifests, and no default, enum or stored shape changes; one risk — the scrub key list now lives as two copies that must not drift.
concern-style-nits
Style verdict: clean — every new helper, regex branch, ceiling and mapping rule in this diff carries a comment that matches the code it describes, and no name, claim or leftover misleads the next debugger.
One doc drift: the new 03-providers openai-oauth row scopes the in-stream quota restriction across all five credits-exhausted codes while the 05-routing row restricts only the four quota codes, leaving account_deactivated's scope contradictory between the two sites.
Style verdict: the slice reads clean except two hand-kept duplications in the scrub grammar — core's mirrored redact.ts patterns carry no twin pointer (drift already bit mid-PR) and the key-name list lives in two regexes one file apart.
credential-redaction (handoff)
Server redactor and its new core twin disagree on ;password=@ DSNs — the server's ={0,2} lacks the twin's key-name lookbehind, so that secret survives server log lines core scrubs, while the new match also eats the host. Core's suite also lacks the quote-stop and port-@ pins its twin has.
openai-oauth-stream (handoff)
Quota restriction and 429-gated HTTP rules match their tests and CLI provenance; account_deactivated stays any-position in-stream, any-status on HTTP. Two minors: on 429 a limit signal beats quota where the stream hook lets quota win, and bare-error account_deactivated is unpinned.
docs-idea (handoff)
Five design docs updated for the #206 fixes; the quota-code scoping, slash-password shape, salvage slot, and redact-then-cut claims all check out. One gap: 08-observability.md's errorClass closed-set row still omits the new openai-oauth:insufficient_quota signal.
dataplane-attempt-log (handoff)
scrubbedQuote correctly serves both log lines and every 08-observability clause holds; the suite is strong. Two minors: a credential straddling the 64 KiB whitespace cut logs a raw fragment when the configured bound is ≥ 64 KiB (main redacted it whole), and a test comment still teaches the deleted…
response-salvage (handoff)
Slots module matches every 09-deployment claim (32-byte status_code ceiling, never status_msg, cap + ≤32-byte lend bound, optional evidence only when small) and every retain/borrow bound I traced holds.

credential-scrubbing-redaction
Adds a slash-password userinfo branch to the API redactor and ports linear-time userinfo/JWT patterns to core, with tests. One coverage regression: core no longer scrubs a long-header JWT preceded by -, which main's unbounded pattern caught.
response-salvage-usage
Adds a bounded base_resp.status_code slot, with lend/block machinery, to the oversized-payload salvage, plus heavy tests; the retained-byte accounting checks out. One added test's name overstates what it asserts, so the tiny-cap code drop is not pinned.
openai-oauth-stream-errors
Adds four Codex quota codes to the in-stream classifier and a 429-only HTTP quota rule, with tests. Risk: the CLI's fifth HTTP quota code is deliberately excluded, so a 429 carrying it still cools down and is retried on a timer.
attempt-log-dataplane
attempt-log.ts now redacts each upstream message before cutting it (shared scrubbedQuote: whole-message pass to 64 KiB, whitespace-bounded past it), and the new suite drives both log lines directly. Logic held up; one added test comment still describes the deleted maxChars + 4096 window.
release-version-bump-docs
Release housekeeping: 2.28.1 agrees across the five manifests, version.ts, the Dockerfile label and the CHANGELOG, and the doc bullets match the doc edits. One docs conflict: two pages give opposite in-stream rules for account_deactivated.
concern-security
Redaction coverage only widens and the salvage stays bounded, but the new userinfo padding guard is bypassed by the pattern's second alternative, so a DSN key=@secret still reaches the boot log.
concern-tests
Every behaviour this PR changes has a test that fails on revert (both log lines, both Codex quota paths, the MiniMax slot, both redactor copies, the core linear-time case); two gaps: an untested envelope/type cell in the new Codex 429 table, and a vestigial 4096-margin premise in the attempt-log su…
concern-api-contract
No signature, type, schema, default or stored shape changes, but the two spec pages this PR edits state Codex status rules the driver does not implement.
concern-style-nits
Three readability debts: the new openai-oauth credits-exhausted qualifier mis-scopes account_deactivated, a test still explains itself with the margin window this PR deleted, and the core scrub now writes its key-name list twice.
release-version-bump-docs (handoff)
Release housekeeping is consistent: five manifests, version.ts and the Dockerfile label all say 2.28.1, and the CHANGELOG link definitions are complete. Two doc lines contradict the PR's own recorded behaviour — the blanket "any JWT" claim and the credits-exhausted type-axis restriction.

It gives way at a full budget, is lent to error evidence and given back if that drops, and a repeatedbase_resporstatus_codereplaces the earlier one (last wins). A malformed code is dropped; any other malformed member fails the salvage as before. Two structural violations at depths 1-2 (a second value under a key; a non-string value where a key or colon belongs) block the code for the whole frame. The scanner is not a JSON validator: other malformed frames can still yield their code. The operator accepted that on 2026-10-14, and the full list is on fix(usage): salvage scanner keeps a repeated parent's stale children; escaped keys miss their slot #209. Fifteen rounds of the two-lens review (correctness and adversarial). Each round's findings were fixed in their own commits, with tests. Final differential fuzz with a sound generator found no input where
redactValueorscrubCredentialsemits more of a credential thanmain(1.6M cases).Known trade-off, recorded on #208: redacting the whole message before cutting it, as #206 item 1 asks, means a log line now reaches further into the message than
main's did. So the userinfo/query-string shapes the redactor already misses (#208 items 1 and 3), and any glued token it misses, can now show wheremaincut the raw text short. That affects both lines for secrets over about 4 KiB, and the attempt line otherwise. No credential the redactor matches is affected. #208 closes these next.
