fix: filter out-of-scope issues before filing - #179

The file-issues node was creating issues that required LLM calls or were outside codegraph's scope, then a later step had to close them. Now filters upfront: no-LLM features, web UIs, vague ideas, and duplicates are excluded before creation. Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>
