Adds docs/rollback.md, the rollback policy referenced by ADR-0003 and listed as a Phase 1 deliverable in docs/roadmap.md. Documents the floating vs immutable tag distinction and the re-pin-forward rollback procedure (immutable fully-pinned tags are never mutated).
