Bug: Nutrition label scanning is unreliable with all providers; "+" menu customization is limited (no 1 group + 1 mode, no reordering of modes)

Steps to reproduce

Issue 1:

1. Choose the native iOS provider or a local Qwen model for recognition.
2. Scan a nutrition facts label.
3. Compare the recognized values with the label.

Issue 2:

1. Open the "+" menu customization.
2. Create a group, then try to add one standalone add-food mode next to it. There is no way to do this.
3. Remove all groups so only standalone modes remain, then try to reorder them. Reordering is not possible.

Expected behavior

1. Nutrition labels are recognized accurately (calories, macros, serving size).
2. Groups and standalone modes can be mixed freely (e.g. 1 group + 1 mode), and standalone modes can be reordered by drag and drop.

Actual behavior

Two separate problems:

1. Nutrition label recognition is poor with every provider I tried: the native iOS recognition and downloadable local models. Values are often missed, misread, or assigned to the wrong nutrient.

2. "+" menu customization is too restrictive:

• I can create groups, but I can't have a layout with one group plus one standalone add-food mode.
• If I remove all groups and keep only standalone add-food modes, I can't change their order.
