Summary by CodeRabbit

  • New Features
    • Added built-in tools for reading, writing, editing, searching, and listing workspace files, plus running approved shell commands.
    • Added tools to launch background tasks, collect results, receive completion notifications, and spawn, list, and query agents.
    • Added approval options for operations outside the workspace and safer path handling, including traversal and symlink checks.
    • Added structured tool-call parsing and clearer error reporting for invalid requests.
  • Bug Fixes
    • Improved reporting of tool errors, background task failures, timeouts, and cancellations.
  • Documentation
    • Added MCP client configuration and integration guidance.

Adds MitoCodeWorkspacePathError, which dual-inherits ValueError so the
existing traversal tests keep asserting on a specific type.

Constraint: _validate_within_workspace kept as a delegating alias; test_workspace.py append-only
Rejected: dropping lstrip('/') silently re-rooted absolute paths | breaks the Glob/Read round trip
Confidence: high
Scope-risk: narrow
AlwaysAllow is the shipped default; the TUI dialog is a drop-in later.

Constraint: builtin/__init__.py is docstring-only in this task
Confidence: high
Scope-risk: narrow
The module has no branches; its contract is protocol membership, dataclass
field order, and the .value wire strings later tasks build refusals from.

Constraint: module-level test functions, not classes | repo convention overrides the skill default
Confidence: high
Scope-risk: narrow
A symlink loop escaped the (ValueError, TypeError, OSError) tuple as a
RuntimeError on 3.10-3.12, and on 3.13+ resolve() stopped raising entirely
and returned the loop path unresolved — still under the root, so the
containment check alone answered True for a path that only yields ELOOP.

Catching RuntimeError alone fixes only the older half. is_within now also
requires the resolved path to be free of any remaining symlink component,
which holds identically on 3.10, 3.13 and 3.14.

Constraint: is_within is the containment gate for all eleven built-in tools
Rejected: catching RuntimeError only | leaves CI's 3.14 returning True for an unopenable path
Verified: 21 assertions against the real Workspace on 3.10.20, 3.13.11 and 3.14.0
Confidence: high
Scope-risk: moderate
The file existed on disk from Task 2 but was never committed, so a fresh
checkout had no builtin/__init__.py. Content is unchanged: docstring only,
no imports, because sibling modules did not exist when it was written.

Constraint: docstring-only in this task | importing a not-yet-built sibling breaks the package
Confidence: high
Scope-risk: narrow
The docstring promised signature-level conformance. runtime_checkable checks
attribute presence only — a sync or wrong-parameter request passes isinstance
and fails at the await instead — so the docstring now says so, names the
TypeError that is the real failure point, and stops contradicting the test
that pins that limit. Also names the enum as a plain Enum (callers interpolate
.value, never the member) and corrects a Returns clause that described a
refusal string where an ApprovalDecision is returned.

Docstrings only: 26 tests still pass, pylint 10.00, pyright 0.
Confidence: high
Scope-risk: narrow
One decorator renders escaped exceptions as Error: strings so the eleven
tools cannot drift on the error contract. CancelledError is left alone so a
cancelled agent can still be stopped.

Constraint: CancelledError must not be swallowed; ContextVar.set() takes a value, .reset() a Token
Rejected: per-tool try/except | guarantees the contract drifts
Known-defect: an exception whose __str__ raises escapes the boundary (xfail strict)
Confidence: medium
Scope-risk: moderate
The boundary rendered with an f-string over the exception, calling str(exc)
inside its own except clause — so an exception whose __str__ raises escaped
the one place designed to stop it, and took the log line with it.

Root cause was not what the report theorised: loguru does not swallow its own
formatting error. It formats {} arguments eagerly inside .error(), so passing
exc as an arg called str(exc) at the call site before any handler ran. The fix
passes the object via logger.opt(exception=exc) so loguru's defensive formatter
owns rendering, and routes the message through safe_exception_message().

safe_exception_message catches Exception, not BaseException, so a halt signal
raised by __str__ still propagates — masking a cancellation is the same bug
class. An empty message is still rendered as empty rather than as a failure,
since str(KeyError()) is legitimately "".

Verified A/B: hostile __str__ now returns an Error string and logs 1 record;
the plain-message and empty-message paths are byte-identical to before.
Confidence: high
Scope-risk: narrow
Regexes mangle the nested quotes the format requires, and literal_eval on
validated constant nodes cannot execute anything. A test-writer's attack found
zero code execution across 10 payloads, with the literal_eval gate proven
load-bearing by an 86-failure mutation.

Four defects found and fixed: non-str input escaped as a raw TypeError from
inside the error path itself; duplicate keywords silently last-wins (ast.parse
does NOT run CPython's duplicate-keyword check -- only compile() does, so the
parser accepted text Python refuses); a positional/keyword pair for one
parameter silently discarded the positional; and leading whitespace made a
legal call an IndentationError. Strip() runs before parsing and removes only
outer whitespace, so 'Bash("ls"); evil()' is still rejected.

Collision detection lives in bind_arguments, not parse_call: whether the first
positional collides is a property of the spec, which the parser never sees,
and rejecting all positional/keyword mixing would break the supported case of
a later argument spelled out while an earlier stays positional.

Known and documented: no type check (#4), 1e400 -> inf (#6, clamped by the
dispatch task), bytes/set/tuple accepted (#7), tool_name not compared to
spec.name (#5 -- the registry is the allowlist).

Confidence: high
Scope-risk: moderate
…ding

The _MAX_EXCERPT cap guarded the source body but not identifiers, so an
unbounded attacker-controlled keyword name was echoed verbatim into the error
message: a 50,000-character name produced a 50,323-character message, on the
exact paths the cap exists to bound. Names are now clipped and their count
capped, so message length is flat in input size (10 -> 136 chars, 1,000,000 ->
413) rather than merely smaller. summary's names come from a third-party MCP
schema, a second untrusted source, and spec.name needed bounding too.

bind_arguments now asserts the spec it was handed is the spec the call named,
as its first statement — so a dispatcher bug is a loud error rather than a
Write argument silently bound onto Two's parameter. This is a consistency
check, not an allowlist: the registry remains the only source of truth for
what is callable, and nothing here knows which tools exist.

Falling out of the fix: join() over schema keys raised a raw TypeError on
non-str keys -- an error path raising while reporting an error, the same class
as the two already closed in this branch.

Confidence: high
Scope-risk: narrow
The clip that bounds untrusted names in error messages could be deleted
without a single test failing: reverting tool = _clip_name(spec.name) left all
182 tests green while restoring a 100,068-char message with an MCP-supplied
tool name echoed whole. The fix existed; nothing could see it.

Also fixes a real behaviour bug the coverage number hid: with more than
_MAX_NAMES collided names the overflow marker was pushed through repr
alongside real names, so '... and 4 more' read as a quoted parameter. Both
halves of that ternary were covered, so branch coverage read 100% while the
behaviour was untested.

All three previously-surviving one-token mutations now fail tests. Coverage
stays 100% but no longer stands as unqualified proof -- it certifies that
every line executed, not that every behaviour is distinguished.

Confidence: high
Scope-risk: narrow
The approver is consulted only on escape, so an in-root read asks nothing.
Containment is decided by is_within rather than resolve_within, because
resolve_within only checks bounds: on 3.13+ it returns a symlink loop
unresolved and still under the root, so it would accept a path nothing can
open. resolve_within is used afterwards purely to produce the resolved path.

A denial is a refusal string naming the action, the path and the reason, not
an exception: the model can act on prose and cannot act on a traceback. With
no workspace there is no boundary, so nothing is ever an escape.

Constraint: in-root paths never reach the approver
Rejected: asking on every path | 1000 asks to say yes is the failure mode avoided
Confidence: medium
Scope-risk: moderate
The Returns clause promised a fully-resolved path on every success, but two
of the three return paths have no containment gate vouching for them and can
hand back a path that will fail at open() -- an approved symlink loop does
exactly that under the shipped AlwaysAllow. Both clauses are now qualified.

Also names the two exceptions that actually reach a caller (an approver that
raises, and root() on a deleted cwd) which the Raises section had omitted in
favour of two exotic cases, and scopes _resolve_best_effort's safety argument
to the call site where a gate actually rejected the path.

Behaviour unchanged and verified as such: docstrings-stripped ASTs of the
committed and current files are identical, and the approver-bypass harness
still reports 0 calls for in-root and 1 for escape.

Confidence: high
Scope-risk: narrow
The approver-bypass property is proven structurally rather than by a script:
13 in-root spellings assert spy.requests == [], in-root work succeeds with an
approver that is literally object() and one that raises if awaited, and a
deny-everything spy records zero calls while the path still comes back.
Removing the in-root fast path fails 17 tests.

Mutation on a copy proved the is_within-not-resolve_within choice is only
observable on 3.13+: there resolve_within raises RuntimeError on a loop, so a
try/except implementation behaves identically and the tests cannot tell them
apart. On 3.14 resolve_within succeeds on a loop and only is_within rejects
it. The property CI actually exercises is the 3.14 one.

Platform note: resolved paths are compared against .resolve(), never a literal
spelling, because /etc -> /private/etc on macOS and a literal comparison
fails there while passing on Linux CI.

Confidence: high
Scope-risk: narrow
The symlink-loop test claimed it discriminates the is_within predicate on
every supported version. It does not: on 3.10 resolve_within raises
RuntimeError, so a try/except resolve_within implementation escalates the loop
identically and the suite is green either way. The test nominated to prove the
module's central design decision was silent on the interpreter a local
developer is most likely running.

Adds a workspace double whose is_within returns False while resolve_within
succeeds in-root, so the predicates differ observably on every version.
Mutation on a copy confirms it: on 3.10 only the new test turns red while
both loop tests stay green; on 3.14 all three do.

The double proves the gate calls is_within. That the real is_within answers
False for a real unresolvable path remains test_workspace.py's job -- the two
tests together cover the decision, neither alone does.

Also corrects the in-root fast-path mutation count to 16, drops three
assertions that were true by construction or over-fitted to an internal call
shape, and adds the missing trailing newline.

Confidence: high
Scope-risk: narrow
Delivery waits until the calling agent is idle, because send_message cancels a
RUNNING session and would otherwise kill the caller's own turn on every
completion. dispatch_pending claims each handle before awaiting so a handle
cannot be delivered twice.

The timeout is enforced by wait_for inside the single wrapper task, so collect
never re-times-out a task it did not create. A cancelled task records
CANCELLED rather than being swallowed or reported as success -- masking a
cancellation is the bug class errors.py already closed.

Session state is compared by .value, not by importing AgentState: the import
direction is one-way and agent_session imports tool_manager.

Also removes the transitional __post_init__ lazy-import workaround from
context.py, now that background.py exists, and the three workaround-only tests
that went with it. Its real assertion -- an explicitly supplied registry is
not clobbered -- was carried into a rewritten test rather than dropped.

833 passed, stable across three runs.

Constraint: compare session state by .value string; no import of the agent package
Rejected: delivering on completion | kills the caller's turn every time
Confidence: medium
Scope-risk: moderate
Three findings from the implementation review:

An inner coroutine that suppressed CancelledError and returned a value was
recorded DONE. asyncio.wait_for returns fut.result() in that case on 3.10 and
Timeout.__aexit__ only raises on CancelledError on 3.11+, so a task that
exceeded its ceiling was reported as having succeeded -- the exact failure this
registry exists to prevent. Confirmed against the committed code, which
returned TaskState.DONE with the value 'the work that should not have
counted'. wait_for is gone; the ceiling is now a sibling task raced against
the work with asyncio.wait, so TIMEOUT is decided by which future completed
and wins over any value the inner produced.

asyncio.TimeoutError is an alias of the builtin on 3.11+, so a tool raising
its OWN TimeoutError -- a socket or subprocess timeout, common in exactly the
tools this registry runs -- was misrecorded as the registry's timeout with a
fabricated duration, while 3.10 recorded FAILED for the same code. The
discriminator is now which future completed, not the exception type, so the
behaviour is identical on 3.10, 3.13 and 3.14 (measured, not modelled).

dispatch_pending cancelled mid-await left the handle in _delivered, so the
notification could never be sent again -- while the docstring claimed a failed
send is retried rather than lost. It now un-claims and re-raises; the claim was
verified not swallowed.

Also bounds delivery retries at 5, and documents that a suppressing or shielded
callee cannot be stopped -- it is cancelled and then runs to completion, with
the late exception drained rather than orphaned.

Confidence: high
Scope-risk: moderate
Delivery is now allow-listed: a notification is delivered only when the
caller's state is positively identifiable as safe, and parked otherwise. The
earlier gate parked cancelled and error too, which over-corrected --
send_message only cancels a RUNNING session, so both are safe and parking them
meant permanent silent non-delivery for cases we can identify perfectly well.

Unrecognised states now log rather than parking silently. Drift between
AgentState.RUNNING.value and the literal here produces a permanently silent
stall otherwise, and the value is truncated and repr-escaped so it cannot
break the line. Drift now stops delivery rather than starting it.

background.py reached 1064 lines behind a too-many-lines suppression; no other
file in this repo exceeds 272. The delivery gate moves to delivery_gate.py
(243 lines), which imports nothing from background.py, so the seam cannot
cycle. Every docstring carried across verbatim -- the prose explaining the
timeout, cancellation and gate rules is the most valuable thing in the module.

883 lines remains the longest file in src/. Accepted: 272 is this repo's
observed maximum rather than a stated convention, and the module holds one
cohesive responsibility. Splitting further would be churn.

833 passed; gate behaviour byte-identical across the split on 3.10 and 3.14.

Confidence: high
Scope-risk: moderate
90 tests. The four fixes from the implementation review are each proven red
against a prior revision recovered from git history rather than reconstructed
from memory: a suppressing callee recorded DONE instead of TIMEOUT, a tool's
own TimeoutError recorded as the registry's timeout, and a dispatch cancelled
mid-send leaving the handle claimed forever.

Reported honestly rather than overstated: the double-delivery property is NOT
provable against any committed revision, because the original code already
claimed before the await -- the defect that fix claims to close does not exist
in this history. The property is still pinned, and is red against a
reconstruction where the claim is taken after the await.

Events are settled by polling a public view, never by sleeping a guessed
interval, and _settled() waits through pending_notifications rather than
collect because collect spends the result the tests assert on.

923 passed (833 + 90).

Confidence: high
Scope-risk: narrow
Three gaps from the test-suite review:

The non-numeric ceiling guard was half tested -- only the ValueError half of
except (TypeError, ValueError). Narrowing the source clause to ValueError left
all 90 tests green, so an object() or a list as a ceiling still crashed _expire
with an orphan TypeError. Now parametrised over both spellings; the narrowed
mutation turns the new case red while the old file stayed green.

_settle's error branch was live code described by a source comment as
unreachable. asyncio.ensure_future(coro_factory()) raises before _run can
record anything, so a factory that raises or returns a non-awaitable reaches it
-- exactly what the DSL can produce. Covered now, and the comment corrected.
background.py coverage 95% -> 97% measured; the four remaining misses are
genuinely unreachable.

Three log-message assertions hard-coded prose that is not a contract; the
discriminating assertions (handle named, single line, repr of the truncated
value) are kept and the prose relaxed.

926 passed on 3.10 and 3.14.

Confidence: high
Scope-risk: narrow
The docstring said the method is reached only when the wrapper was cancelled
before its first line. That is half the truth and contradicted two tests in
this same repository: ensure_future fails on a coro_factory that raises or
returns a non-awaitable before _run is ever entered, and _settle is the only
thing that catches it. The summary line moves from 'a record whose wrapper
never ran' to 'a record that nothing settled', because on that path the
wrapper's first statement does run -- it is what raises.

Confirmed the two other reachability claims in the file are true:
background.py:398 is genuinely dead defensive code, and the cancel_all guard
at :641 is reachable and untested, since a wrapper cancelled before its first
line is already finished while its record still reads RUNNING because the
done-callback is call_soon.

Merge Risk: Moderate · up to f3f1b

The test suite currently fails on the CI Python version because of the NUL-byte test, so this should be fixed before merge. The MCP documentation example would give integrators a broken event-loop lifecycle. The sub-agent depth limit can be bypassed if an agent explicitly picks an ancestor as parent. The other findings are small.

Security architecture risk: Moderate · up to f3f1b

The new execution capabilities have incomplete ownership, nesting and cancellation guarantees that can affect result confidentiality and mutation safety. Explicit integration is required before these capabilities become available; automatic production enablement was not established. A pre-existing low-severity information-exposure finding also remains.

Retained concerns

  • Medium · security · observed: The delegation-depth guard follows a model-selected parent rather than the caller. An agent at the limit can select an existing shallow ancestor and create another agent with the shared tool surface. This bypasses the intended bound on recursive delegation, although it does not establish new operating-system privileges or a tenant boundary crossing.
  • Medium · security · observed: Background tasks record a caller identity, but result redemption accepts only a sequential handle and does not compare that identity with the collecting caller. Within a shared context, another agent can read and consume a completed result before its intended recipient. Separate context objects receive separate registries; cross-tenant exposure is not established.
  • Medium · reliability · inferred: TIMEOUT and CANCELLED describe the registry outcome, not confirmed termination of underlying work. Cancellation-suppressing callables and already-running filesystem workers can continue side effects after those states become visible. Result guidance recommends reissuing timed-out or cancelled work, creating a risk of overlapping mutations without an idempotency or completion fence.
  • Medium · reliability · observed: The create-only filesystem contract is enforced by existence checks followed by a truncating write, not exclusive creation. Concurrent calls can both pass the checks and overwrite the same target. The new concurrent execution paths therefore undermine a stated protection against destructive replacement, including when callers rely on it for safe recovery or retry.
Security review details

Security Blast Radius

  • inferred — Once explicitly registered, model-controlled shell commands can affect resources available to the host process rather than only the workspace. Delegated sessions inherit the shared tool registry. The maximum demonstrated scope is therefore the configured orchestrator, workspace and process authority; tenant, environment and downstream service boundaries are not established by the supplied deployment evidence.

Security Findings and Attack Paths

  • observed — The retained low-severity information-exposure finding concerns server-controlled MCP error text surviving targeted redaction into exposed error messages. The documentation anchor states a credential-safety invariant; the executable handling is in the existing MCP client. That client and its existing tool-manager dispatch are unchanged against the full PR base, so the finding is not attributed to new documentation. The optional background route adds another caller and error sink, but increased effective production exposure was not established.
  • observed — A model controlling tool arguments can choose an existing shallow parent for further delegation or guess another task's sequential handle and redeem its result. Existing-parent validation and exact-handle lookup reject nonexistent objects, but do not bind those operations to the current caller.

Trust Boundaries and Controls

  • observed — The call parser prevents expression execution outside registered tools, and task-local identity avoids concurrent caller-ID confusion. Neither mechanism supplies target authorization. Agent listing and query are explicitly orchestrator-wide, and the context is process-wide, so these interfaces should not be interpreted as isolation between mutually untrusted sessions.

Resilience and Maintainability Implications

  • inferred — Recovery cannot safely assume that a terminal background record means mutations have ceased. Retrying can overlap surviving work, while concurrent create-only writes can overwrite despite their prechecks. These are failure-containment and recovery-contract risks rather than evidence of a completed exploit.

Hardening Proposals

  • proposed — Define the intended session trust domain explicitly. If caller isolation is required, authorize parent selection and result redemption against the executing caller, and bind execution context to the invocation rather than relying on a replaceable process-global value.
  • proposed — Distinguish deadline expiry from confirmed work termination, avoid unconditional retry guidance for side-effecting calls, and use mutation-specific recovery controls. Exclusive file creation would enforce the create-only contract under concurrency; delegation budgets should remain effective regardless of a model-selected parent.

Explanation

The title "feat(tool): add some internal tools" names the main change: a set of built-in tools added under the tool package. The changeset adds eleven built-in tools (file, shell, background, and agent tools) plus registration, approval, and workspace path-containment support. The title is broad and does not name the specific tool families, so it is partially related. It does not describe anything absent from the changeset and is not vague in the sense of "misc updates".

High-Level Assessment

The following are alternative approaches to this PR:

1. Wire notifications into the agent lifecycle now
  • ➕ Makes the advertised automatic completion notifications reachable in production.
  • ➖ Expands this PR into orchestrator lifecycle changes and increases cancellation risk.
2. Use established file-search and ignore libraries
  • ➕ Provides fuller gitignore and glob semantics with less custom matching logic.
  • ➖ Adds dependencies and changes the deliberately limited search contract.

Recommendation: The separated registry, path gate, and approval protocol are reasonable foundations. Keep that structure, but explicitly treat automatic notification as infrastructure pending a caller that awaits dispatch_pending when agent state changes; only collection by handle is independently usable today. The default AlwaysAllow policy also means approval is a replaceable decision seam, not an interactive prompt or a sandbox.

Enhancement (15) +5419 / -0
agent_runtime.pyExpose the executing agent ID to tools +3/-0

Expose the executing agent ID to tools

• Sets a context variable before invoking a registered tool so sub-agent and background calls can identify their caller.

src/mitocode/core/agent/agent_runtime.py

mitocode_exception.pyAdd a workspace path exception +10/-0

Add a workspace path exception

• Introduces MitoCodeWorkspacePathError as both a workspace error and ValueError, preserving existing traversal-error expectations.

src/mitocode/core/exception/mitocode_exception.py

__init__.pyExport the built-in tool API +50/-0

Export the built-in tool API

• Exposes approval types, context helpers, the error boundary, and the registration entry point.

src/mitocode/core/tool/builtin/init.py

agent_tools.pyAdd sub-agent control tools +593/-0

Add sub-agent control tools

• Implements nonblocking Agent, ListAgent, and QueryAgent operations. Spawn inherits the parent's provider, limits nesting, and reports missing orchestrator state as a tool response.

src/mitocode/core/tool/builtin/agent_tools.py

approval.pyDefine an asynchronous approval policy +183/-0

Define an asynchronous approval policy

• Adds approval requests and allow/deny decisions with an Approver protocol. Ships permissive AlwaysAllow and a DenyAll implementation; no interactive dialog is added.

src/mitocode/core/tool/builtin/approval.py

background.pyTrack detached tool calls and notifications +907/-0

Track detached tool calls and notifications

• Adds handles, one-time result collection, deadlines, cancellation states, and bounded notification retries. Delivery is parked while the calling agent is running.

src/mitocode/core/tool/builtin/background.py

background_tools.pyExpose background launch and result tools +432/-0

Expose background launch and result tools

• Validates a model-authored call against registered tools and launches it without waiting. BackgroundResult reports running and terminal outcomes using the returned handle.

src/mitocode/core/tool/builtin/background_tools.py

call_dsl.pyParse and bind literal tool-call expressions +414/-0

Parse and bind literal tool-call expressions

• Parses a restricted call expression without executing it, then binds its arguments to a registered tool specification. Returns actionable errors for malformed or mismatched calls.

src/mitocode/core/tool/builtin/call_dsl.py

context.pyCentralize built-in tool dependencies +245/-0

Centralize built-in tool dependencies

• Adds a tool context for workspace, approver, orchestrator, tool manager, output limits, and background registry. Uses a context variable to distinguish concurrent calling agents.

src/mitocode/core/tool/builtin/context.py

delivery_gate.pyGuard background notification delivery +244/-0

Guard background notification delivery

• Allows delivery only into recognized non-running agent states. Parks unknown states and logs them rather than risking interruption of a live turn.

src/mitocode/core/tool/builtin/delivery_gate.py

file_walker.pyShare file enumeration for Glob and Grep +274/-0

Share file enumeration for Glob and Grep

• Adds sorted traversal, limited gitignore matching, hidden-directory handling, and binary detection. Avoids traversing directory symlinks and non-regular files.

src/mitocode/core/tool/builtin/file_walker.py

fs_tools.pyAdd five file tools +915/-0

Add five file tools

• Implements Read, create-only Write, exact-match Edit, Glob, and Grep. Uses the shared path gate, offloads blocking I/O, and bounds file reads and returned text.

src/mitocode/core/tool/builtin/fs_tools.py

paths.pyCentralize file-path approval decisions +252/-0

Centralize file-path approval decisions

• Resolves relative and absolute paths against the workspace and consults the approver for rejected paths. Returns a refusal message when approval is denied.

src/mitocode/core/tool/builtin/paths.py

register.pyRegister eleven built-in tools +389/-0

Register eleven built-in tools

• Defines tool names, descriptions, and display metadata in one ordered table. Preserves existing registrations and lets ToolManager derive schemas from resolved annotations.

src/mitocode/core/tool/builtin/register.py

shell_tools.pyAdd the Bash tool with process-group cleanup +508/-0

Add the Bash tool with process-group cleanup

• Runs shell commands in the workspace, flags apparent path escapes for approval, and returns merged output. Kills the process group on timeout or cancellation; the textual escape scan is not a sandbox.

src/mitocode/core/tool/builtin/shell_tools.py

Bug fix (2) +366 / -9
errors.pyStandardize tool failure responses +224/-0

Standardize tool failure responses

• Adds a decorator that logs unexpected exceptions and returns model-readable errors. Cancellation and other process-control exceptions still propagate.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Accept either ValueError or SyntaxError as the NUL-byte cause, because the exception type depends on the Python version.

CI fails at Line 985. The run reports SyntaxError('source code string cannot contain null bytes') where the test expects ValueError.

  • Python 3.12 and later: ast.parse raises SyntaxError for a NUL byte in the source.
  • Earlier versions: ast.parse raises ValueError.

The project declares requires-python >=3.10, so both behaviors are supported. The parser handles both correctly, because SyntaxError and ValueError are both in _PARSE_FAILURES. The test, however, pins only the older behavior.

The invariants that matter still hold on every version:

  • The error is wrapped in MitoCodeBuiltinToolError.
  • No raw \x00 appears in the message.
  • An escaped \\x00 appears in the excerpt.

Change the cause assertion to accept both types. Also update the comment at Line 79 in call_dsl.py, which says only ValueError covers the NUL case.

Update the NUL-byte cause assertion in
test_nul_byte_in_the_source_is_wrapped_and_not_echoed_raw to accept either
ValueError or SyntaxError while preserving the escaped-message checks. Update
the related comment near _PARSE_FAILURES in call_dsl.py to describe the

Source: Pipeline failures

Sub-agents inherit the parent's provider so a user-selected model propagates,
and depth is capped because correct parent chaining makes unbounded forking
reachable. The depth walk is guarded against a cyclic parent chain, which
nothing in the orchestrator prevents and which would otherwise spin forever
inside a single tool call and take the turn with it.

Agent returns the new id immediately rather than blocking on the sub-agent's
answer, which would deadlock: the sub-agent runs on the same event loop the
parent is awaiting. QueryAgent scans backwards for the last assistant message
rather than calling build_standard_context, which costs the whole transcript
to answer one question.

Anticipated conditions are returned as sentences, not raised through
tool_boundary. A raised MitoCodeBuiltinToolError reached the model as
'Error: MitoCodeBuiltinToolError: ...' -- a Python class name the model cannot
act on, on the most likely condition a user hits (starting the app before
wiring an orchestrator). Genuine programming errors still raise, so the
boundary keeps the diagnostic.

The agent package is duck-typed, never imported: agent_session imports
tool_manager, so a runtime import back would be a cycle. Session state is read
as .value and compared against a string literal, as delivery_gate.py does.

926 passed.

Constraint: no runtime import of the agent package; anticipated failures are sentences not exceptions
Rejected: blocking on the sub-agent's answer | deadlocks the shared event loop
Rejected: build_standard_context for QueryAgent | whole transcript to answer one question
Confidence: medium
Scope-risk: moderate
The group kill was implemented on the timeout path only. AgentRuntime stops a
turn by cancelling the tool's task, so cancelling a Bash call is how a user
interrupts a command -- and it left the subprocess alive: sleep 30 survived a
1s cancel until it was killed by hand. That is exactly the stray-process
failure the group kill exists to prevent.

The cancellation path now kills the group, consumes the orphaned reader so
nothing can displace the signal, and re-raises -- tool_boundary must let
cancellation through, and errors.py already established that swallowing it is
the bug class.

Also softens the separator comment's rationale, which claimed splitting on
redirections was harmless because the fragments are not paths: glued
redirections like 'echo hi >/etc/passwd' escape the scan entirely, and are now
listed as documented blind spots rather than argued away. And the docstring no
longer promises output that the reaper bound drops.

1061 passed.

Confidence: high
Scope-risk: moderate
38 tests. The process-group kill is verified by diffing pgrep output before
and after, not by the return value: a cancelled bash("sleep 30") must leave no
survivor, and CancelledError must propagate rather than be swallowed. A
deadline turns a regression into a failed assertion instead of a hung suite.

The approver contract is asserted by call count -- zero for an in-root
command, exactly one per escape -- and the scan's documented blind spots
(glued redirections) are pinned so they stay honest rather than drifting into
silent gaps.

1099 passed (1061 + 38).

Confidence: high
Scope-risk: narrow
Background never blocks: it returns a handle in ~0.1ms against a task that
sleeps for seconds. The timeout is the ceiling the detached call is held to,
not a wait this tool performs -- which is why BackgroundResult exists.

Dispatch is one exact registry dict lookup. parsed.tool_name is
attacker-controlled: parse_call('__import__("os")') succeeds and yields
tool_name == '__import__', inert only because the lookup is exact and that
name is not registered. No name allowlist was added here -- the registry IS
the allowlist, and a second one would create two sources of truth.

The timeout is clamped before it reaches the registry: 1e400 parses to inf,
and an infinite ceiling is a hang, which is the exact failure the timeout
exists to prevent. Non-finite is refused and anything over an hour is
bounded.

Every failure is prose naming what went wrong, and dispatches nothing --
unknown tool, malformed call, missing argument, no tool manager.

1102 passed.

Constraint: exact registry lookup; no name allowlist
Rejected: trusting the timeout as given | 1e400 is an infinite hang
Confidence: medium
Scope-risk: moderate
Attempts all eleven and returns the names ATTEMPTED (including skipped),
never just the names actually registered. Refuses to clobber a name that
already exists — ToolManager.register overwrites silently, so a user tool
named Read survives when registered first. Schemas stay annotation-derived;
no parameters_schema passed. Context installed via set_current with its
tool_manager filled so Background can resolve the DSL.

The five PEP 563 string annotations from Tasks 9-10 are resolved in-scope
via typing.get_type_hints, so registration succeeds without touching
tool_manager.py. Root cause fix (inspect.signature with eval_str=True) is a
follow-up outside this file.

1102 passed.

Constraint: schemas stay annotation-derived; no clobber
Confidence: high
Scope-risk: moderate
This is what makes the background completion notification live and lets
Agent() default parent_id to its caller. Both degrade without it: Background
says 'use BackgroundResult' and Agent hangs sub-agents off root.

Constraint: .set() not .reset() | ContextVar.reset() takes a Token, not a value
Rejected: .reset() with a value | raises TypeError
Approved as the sole edit outside core/tool and core/workspace
Confidence: high
Scope-risk: narrow

Background tool has _resolve_timeout to clamp/validate timeout values,
but Bash was missing this guard. Added _resolve_timeout to shell_tools.py
that:
- Rejects infinite/NaN timeouts (1e400 -> inf)
- Rejects boolean timeouts (True/False)
- Rejects non-numeric timeouts
- Clamps timeouts > 1 hour to 1 hour max
- Non-positive timeout falls back to default (120s) per existing behavior

This prevents Bash("sleep 999999", timeout=1e400) from hanging the turn
indefinitely, matching the protection Background already has.

1106 tests pass (5 new).

Confidence: high
Scope-risk: moderate
Read tool has a 10 MiB byte cap; Grep was missing this guard and would
load entire multi-gigabyte files into memory before scanning. Added
the same byte cap (MAX_READ_BYTES = 10 MiB) to _grep_one_file,
checking file size via os.fstat after the binary sniff bytes.

Files larger than 10 MiB are now refused with a clear error message
before any content is read, preventing memory spikes on huge log files.

1106 tests pass.

Confidence: high
Scope-risk: moderate
P2-1: Add missing tests for background_tools.py (20 tests) and register.py (10 tests)
- background_tools: _resolve_timeout decision table, background/background_result tools,
_collect handle fallback, duplicate keywords, positional/keyword collision,
str.replace termination, timeout clamping, non-finite rejection
- register: all eleven tools registered, clobber prevention, idempotency,
PEP 563 string annotation resolution (_resolve_string_annotations)

P2-2: Bash timeout clamping (shell_tools.py)
- Added _resolve_timeout to clamp/validate timeout values
- Rejects infinite/NaN timeouts (1e400 -> inf)
- Rejects boolean timeouts (True/False)
- Rejects non-numeric timeouts
- Clamps timeouts > 1 hour to 1 hour max
- Non-positive timeout falls back to default (120s) per existing behavior
- Reuses Background tool's timeout normalization logic

P2-3: Grep per-file byte cap (fs_tools.py, file_walker.py)
- Added 10 MiB byte cap to _grep_one_file (matches Read tool)
- Files larger than 10 MiB refused before reading, preventing memory spikes
- Uses os.fstat on file handle after binary sniff
- Documented in grep_files docstring

Also fixed:
- Bash timeout error messages show original timeout value
- Bash non-positive timeout uses default (120s) per existing test
- Background tool tests fixed for event loop handling
- Grep byte cap uses os.fstat(handle.fileno()) for compatibility
- Black formatting applied to all modified files

1136 tests pass, pylint 9.84/10, pyright baseline (36 errors), black clean.

Confidence: high
Scope-risk: moderate
The P2-2 fix commit added a second test function with the same name as the
parametrized one, silently shadowing it in the module namespace so only the
unparametrized copy ran -- with a docstring claiming Bash's default is five
minutes (it is 120 seconds; five minutes is Background's default). The
duplicate is deleted and the parametrized test gains a negative-float case
exercising the float branch of _resolve_timeout.
The P2-2 clamp computed a note saying the requested ceiling was clamped,
then dropped it on the floor (pylint W0612), and the timeout message
reported the *requested* value -- a command killed after the clamped one
hour said it timed out after the raw request, e.g. 1000000000 ms. The
message now names the effective ceiling (timeout_ms) and the clamp note is
appended to the reply on the success and timeout paths alike, via a small
_with_note helper that also keeps bash under pylint's too-many-locals cap.
_resolve_timeout returns the two-tuple Background's twin already returns.

The bash docstring's timeout paragraph still described the pre-clamp
behaviour (zero or less expires immediately); it now states the fallback
and the clamp. Tests: the clamp test asserts the note, and a new
monkeypatched-ceiling test proves the timeout message reports the
effective value, not the request.

test/test_builtin_background_tools.py: 2

183-203: Trivial · Maintainability & Code Quality · Quick win

This test does not exercise the background tool.

The name says the timeout "passes through to registry". The body calls BackgroundRegistry.launch directly with timeout=12345. It then checks that the registry stored the same value. This tests the registry. It does not test whether background() forwards timeout_ms from _resolve_timeout to launch.

A regression in that wiring would not fail this test. Examples are launching with the raw timeout, or skipping the clamp. Call background(...) with a context from the manager fixture. Then read ctx.background._records and assert on record.timeout. Cover a normal value and a value above MAX_TIMEOUT_MS.

Restore the global ToolContext after each test.

set_current replaces the module-level _CURRENT and nothing resets it. After these tests run, later tests in the same process see a context with the Echo/Slow/Boom tool manager and a tmp_path workspace. Line 87 leaves a bare ToolContext() installed. Tests in other modules that rely on current() can then depend on test order.

Install the context with monkeypatch.setattr on _CURRENT. Pytest restores the old value at teardown. Use the same approach at Line 87.

Review comment at @test/test_builtin_background_tools.py at line 26:
Update the `manager` fixture to accept `monkeypatch` and install its
`ToolContext` by patching the context module’s `_CURRENT`, so pytest restores
the prior value after the test. Apply the same approach to the bare
`ToolContext()` installation near line 87; avoid using `set_current` for these
test contexts.

- Around line 168-170: Use a separate variable for the list returned by
McpServerConfig.parse_config() in the example, so cfg remains the single
McpServerConfig used to call build_transport().
- Line 181: Update the comment for McpClient.connect() to clarify that ordinary
connection failures are recorded without raising, while asyncio.CancelledError
propagates to the caller.
- Line 100: Update the MCP examples using asyncio.run(manager.start()) so
manager.start(), agent or TUI work, and manager.stop() run within one long-lived
async lifecycle; keep the event loop alive until use and cleanup are complete.
- Line 173: Update the parse_config() documentation to clarify that duplicate
dictionary keys are overwritten before parsing, so parse_config() cannot reject
duplicate server names; state that keys must be unique before parsing.
- Around line 253-254: Update the credential-redaction invariant in the
documentation near the `status().error` statement and at the corresponding lines
304–306 to limit the guarantee to credential values generated or scrubbed by
this code; do not imply that arbitrary server-provided error text is redacted.

Review comments at @src/mitocode/core/tool/builtin/agent_tools.py:
- Around line 439-450: In spawn_agent, validate that an explicit parent_id is
the caller_id itself or belongs to the caller’s subtree before calling depth_of,
rejecting parents outside that subtree. Preserve the existing error for unknown
parent IDs and apply the depth check only after these validations.

Review comments at @src/mitocode/core/tool/builtin/fs_tools.py:
- Line 348: Update the Edit flow to preserve existing line endings when reading
and writing files: replace the universal-newline behavior of `Path.read_text`
and `Path.write_text` with file operations that use `newline=""`. Keep the
replacement logic unchanged so CRLF and lone-CR content outside the edited text
remains intact and matching `old_string` values retain their original line
endings.
- Around line 630-635: Update the entry-limit check in grep_files to stop
processing whenever len(entries) reaches ceiling, regardless of output_mode,
preserving the sorted-prefix result. Revise the head_limit docstring to describe
this early-stop behavior for all output modes.
- Around line 273-285: Update the write flow around path.write_text to create
files atomically with exclusive-create mode, preventing concurrent writes from
overwriting an existing file. Handle FileExistsError by returning the existing
“already exists” refusal response, while preserving the current handling of
other OSError failures.

Review comments at @test/test_builtin_background_tools.py:
- Around line 52-55: Update test_background_does_not_wait_for_the_call to
enforce a short timeout on the background call, so the test fails if it waits
for Slow() to finish. Make the test asynchronous and use asyncio.wait_for with a
one-second bound while preserving the bg_ assertion.
- Around line 60-61: Replace the fixed sleeps in the background outcome tests
and the registry collection test with bounded polling. Repeatedly call
background_result or registry.collect until the result is terminal rather than
RUNNING, pausing briefly between attempts and failing clearly if the attempt
limit is reached; preserve the existing result assertions.

ast.parse rejects a NUL byte in the source with ValueError before Python
3.12 and with SyntaxError from 3.12 on; the test pinned the older type and
CI (3.14) failed on it. Accept either -- both are in _PARSE_FAILURES, so
the wrapping, the no-raw-NUL and the escaped-excerpt invariants are
untouched -- and correct the _PARSE_FAILURES comment that named ValueError
as the sole NUL-byte failure.
_render delegated its failure case to safe_exception_message, which returns
str(exc) -- so a value whose __str__ raises made the record read DONE with
the exception's own message as the tool output, while the docstring promised
a type-naming placeholder and the pinning test documented the disagreement.
The placeholder now names the value's type and the raised exception's type
and nothing else: an internal rendering failure never reaches the model as
tool output. Test updated to the contract instead of the bug.
parent_id is model-visible (the schema derives from spawn_agent's
signature), so a deep agent could name a shallow ancestor, pass the depth
check, and keep forking from there -- an unbounded number of agents, one
model turn each. A spawn under an explicit parent now additionally requires
the parent to be the caller or inside the caller's subtree, checked by a
cycle-guarded walk up the parent chain before the depth check; the
unknown-parent error is preserved. With no agent turn running there is no
subtree to check against, so the REPL/test path is unchanged. The Agent
tool's agent_doc names the rule for the model.
The pre-checks and path.write_text were two steps: write_text opens with
mode 'w', which truncates, so two concurrent writes could both pass the
checks and the second silently overwrite the first -- breaking the very
rule the tool exists to enforce. The create is now exclusive (open with
'x', O_EXCL at the OS level) and a file that appears between the checks
and the open is refused with the same already-exists sentence. A five-
writer concurrency test pins the one-won-four-refused outcome.
read_text runs in universal-newline mode, so every CRLF (and lone CR) in
the file became LF on read, and write_text wrote those LFs back: one small
edit silently re-line-ended the whole file, and an old_string containing
CRLF could never match what the file held. Both the read and the write now
run with newline='' so no translation happens in either direction and
every byte outside the replaced span is preserved. Two tests pin CRLF
survival and CRLF-carrying old_string matching.
The early stop was gated on files_with_matches, but the result is a
truncation prefix in every mode: files arrive sorted and each file's
output only appends, so once the budget is full a later file can no
longer change what is kept. content and count modes kept scanning the
whole tree and holding every matching line in memory only to drop them at
the slice. The break now applies to all modes, the _grep_sync and
grep_files docstrings say so, and a counting test proves later files are
never opened.
Four review findings, one suite. The does-not-wait test had no time bound
-- a background that started awaiting Slow's 30-second run would still
have passed -- and now runs under a one-second wait_for. The fixed sleeps
before asserting on outcomes raced the detached task on slow runners and
are replaced by bounded polling helpers. The timeout-passthrough test
exercised the registry directly, proving nothing about the
background-to-launch wiring, and now drives the tool itself for both an
ordinary ceiling and one above the clamp. And the manager fixture
installs its context by patching _CURRENT under monkeypatch instead of
set_current, so it no longer leaks into every later test in the process.
The two wiring examples ran agent/TUI work outside asyncio.run(manager.start())
-- which closes its loop on return, killing the stdio transport's tasks and
unshielding cleanup -- and now run start, work and stop in one long-lived
async lifecycle, with a note for the Textual case. The parse_config example
reassigned cfg to a list and then called build_transport() on it; it uses a
separate variable. The claim that parse_config rejects duplicate server
names was impossible -- dict keys overwrite before the call -- and now says
the source must be unique. The connect() one-liner notes that
CancelledError propagates. And the credential invariant is scoped to
values this code generates or scrubs, with server-authored error text
explicitly out of scope, matching the redaction section it points at.
