Polish Camera Bubble as a proper macOS app

What changed

• adds an original Camera Bubble app icon and bundles it as .icns
• adds a persistent menu-bar controller
• adds show/hide, timer, reset, mirroring, always-on-top, privacy settings, About, and Quit actions
• remembers window size, position, and camera mirroring
• adds a visible hide control to the floating bubble
• bumps the app to v1.1.0 and updates README documentation
• keeps the existing local-only camera and no-networking privacy model
