Record post-Phase-67 outcome/report/eval audit

Summary

• add a current-code audit for the post-Phase-67 bounded-world outcome/report/eval generalization candidate
• record that selected-world proof is current for fog-harbor-east-gate, museum-night, and library-rain, but does not claim future-world readiness
• keep the queue paused and explicitly do not open Phase 68 from this audit
• add regression coverage for the audit wording, source evidence, runtime-created/future-world boundary, safety redlines, and current-doc pointers

Review

• read-only evidence auditor found that existing repo truth does not allow opening Phase 68 directly and recommended a narrow current-code audit
• read-only context explorer confirmed selected-world report/eval evidence is current while runtime-created/future-world readiness remains unproven
• final read-only safety/evidence review found no blockers and no need for ADR, status:needs-adr, or risk:safety
