Add Manchester chapter page and nav links - #30

Adds a new /manchester chapter page modelled on the existing chapter pages (London, Glasgow, Oxford, Leicester), with a Manchester card on the homepage chapters grid and a Manchester entry in the Chapters dropdown across all top-level pages.
