Meta: failure modes Foreman should have caught #1

This is the one open thread. Post here instead of opening a new issue,
unless you have a specific bug.

Foreman exists because website builds fail in the same few places. The
playbook currently intercepts nine of them. It is certainly missing some.

If your build broke, tell me where. The most useful report is not
"it didn't work." It is: the site was live, and then this went wrong,
and Foreman never asked me about it.

Format, roughly:

  • What you were building, in one line
  • Which phase you were in, or "after shipping"
  • What broke
  • What question would have prevented it

That last line is the whole point. A good answer becomes a gate, a
constraint in the brief, or a bullet in the failure-modes list, with
credit in the changelog.

Also useful here

  • Agents other than Claude that you ran this on, and whether the phase
    gates actually held
  • Places the agent narrated the playbook at you instead of running it,
    which is the failure mode I most want to kill
  • Hosts and registrars whose DNS panel does not match the instructions
  • Sites you shipped with it, which I will link in the README
