feat: isolate lifecycle telemetry account streams

Summary

• partition new live execution facts by stable Cloud Run/runtime stream identity
• refuse to combine multiple account streams into one strategy return curve
• add an explicit monitor stream filter and document strict monitoring usage

Safety

• telemetry and monitoring only; no broker, order, strategy, or promotion authority
• legacy unscoped records remain readable but cannot be silently merged with multiple scoped streams

Validation

• ruff check for changed lifecycle modules/tests
• 35 focused lifecycle tests passed
