Include reviewer OIDC retirement in exact cleanup bundle

Summary

• extend the protected-base cleanup bundle by six exact OIDC/deployment regression paths
• keep the normal 2,000-line limit, the cleanup-only 3,000-line limit, and the exact three deletions unchanged

Verification

• simulated the full 22-path cleanup against the protected-base gate: 2,724 lines, zero issues
• python3 -m pytest -q tests/test_gate_codex_app_review.py
• python3 -m json.tool .github/codex_auto_merge_policy.json
• git diff --check
