fix: preserve confirmed exit evidence across adapter health checks - #604
Health checks now report a confirmed process exit as a failure. Previously, a lost health connection could hide that evidence and leave the result as unknown.
Details
Examples on Linux and macOS, for an adapter that supports live:
Example
Health result
The adapter host exits while a ready check waits for a response
failed/agent_exited, with runtime.state = failed
The health connection fails, but the process exit cannot be confirmed
unknown
The selected runtime or session is replaced during the check
unknown/observation_changed
Checks run live first. A confirmed host exit ends the check at that stage, even if the adapter does not support the requested higher level.
The change also preserves failures reported by native processes and closed sessions. Another thread using the process registry no longer hides confirmed exit evidence. Test connection waits have a time limit, and the guide explains Kilo's session checks more precisely.
Health checks leave the runtime unchanged. A host-exit report does not confirm that every child process has stopped. No API or configuration changes.
Validation
Full Python suite before rebasing: 3,341 passed, 147 skipped on macOS with Python 3.12.
Rust workspace: 321 passed with tests run in sequence after an existing CLI temporary-directory collision. The Rust core also passed 257 tests in parallel.
TypeScript suites and package checks passed. This docs update also passed 54 Kilo tests, with 1 optional skip, and the existing all-adapter exit regression.
After rebasing: 64 documentation and NOOA tests passed.
just docs, compilation, formatting, Ruff, pre-commit, and patch checks passed. Docs and Clippy retain existing warnings.
Security review and credential scans found no introduced security issues or leaked credentials.

Summary by CodeRabbit
Bug Fixes
Health checks now report confirmed adapter-host or agent exits as failed/agent_exited, including when an exit occurs during a check.
Transient probe failures remain inconclusive rather than being mistaken for confirmed exits. Checks also distinguish closed sessions and changed observations.
Confirmed exit results persist across later checks, and health checks do not stop or close sessions.
Documentation
Clarified how health checks report confirmed exits, probe failures, and changes to runtime or session identity, including platform-specific behavior.
An error occurred during the review process. Please try again later.
Walkthrough
Fabric runtime checks now report confirmed owned-host exits as failed/agent_exited. Adapter health checks preserve confirmed native exits and session closures while distinguishing changed observations from inconclusive probe errors.
Platform process checks and runtime tracking report confirmed exit status or unknown evidence. Tests cover live processes, missing identity, and changed identity.
Apply host exit evidence to runtime checks crates/fabric-core/src/check.rs, crates/fabric-core/src/runtime.rs, tests/e2e/test_hermes_checks.py
Local runtime checks inspect host status before and after the adapter probe. Confirmed exits produce failed runtime health results. Tests cover process exit during probes, registry conditions, and host-level reporting.
Preserve adapter exit and closure results adapters/python/codex/src/nemo_fabric_adapters/codex/adapter.py, adapters/typescript/{cline,droid,kilo,qwen}/src/*-sdk.ts, adapters/typescript/{cline,droid,kilo,qwen}/test/*
Adapter checks distinguish confirmed process exits and session closures from changed observations and other probe errors. Tests cover resolved and rejected probes.
Additional tests check runtime result preservation, exit detection, closure handling, timestamps, and error sanitization. Documentation describes health outcomes and their limits.

# Python Test Style
- `tests/conftest.py` contains a `restore_environ_fixture` fixture that restores the environment variables to their original state after each test, it is defined with `autouse=True` so it is automatically applied to all tests. If you need to modify the environment variables in a test, do so using `os.environ` and the fixture will restore them after the test completes. There is no need to use `monkeypatch.setenv` to modify environment variables in tests.

Check the active session identity before reporting native-process exit.
When an active response has a different response.data.id and the server is no longer alive, the current branch returns exited() before it checks the session ID. Kilo therefore reports failed/agent_exited for a changed observation. Move the exit check after response and session-identity validation.

Retain exit evidence for timed-out hosts.
When a host invocation times out, invalidate_timed_out_local_host removes the process-registry entry but leaves the runtime check binding active. local_host_has_exited then returns None, so a later live check can return Unknown/ProbeError instead of Failed/AgentExited.
Keep the process entry until the runtime binding is invalidated or replaced.
