feat: complete hot-reload workbench review

Summary

• complete the client hot-reload lifecycle with disposal, rebinding, and state continuity
• hide patch metadata in review view while preserving syntax highlighting and precise inline change ranges
• synchronize light/dark appearance with DSH and use layered OpenCode-style diff surfaces
• preserve UTF-8 text across Git review output and add regression coverage for review, HMR, theme, and package behavior

Validation

• node --test tests/*.test.mjs — 18 passed
• TypeScript no-emit check — passed
• package build and package verification — passed
• pnpm check remains affected by the existing Windows EPERM temporary-file permission issue
