Docs: move the long reference sections out of CLAUDE.md - #151

CLAUDE.md loads into every session and subagent that touches this repo. The long per-topic reference sections move verbatim to CONVENTIONS.md (read on demand), and CLAUDE.md keeps a one-line-per-topic index pointing at them. No rule changed.

CLAUDE.md loads into every session and every subagent that touches this repo, so its size was paid on every turn. The long reference sections move verbatim to a sibling file (read on demand) and CLAUDE.md keeps a one-line-per-topic index pointing at them. No rule changed; a script checked the moved text plus the kept text reproduce the original exactly.
