Docs: move the long reference sections out of CLAUDE.md - #112

CLAUDE.md loads into every session and subagent that touches this repo. The long per-topic reference sections move verbatim to CONVENTIONS.md (read on demand), and CLAUDE.md keeps a one-line-per-topic index pointing at them. No rule changed.

The long reference sections move verbatim to a sibling file (read on demand) and CLAUDE.md keeps a one-line-per-topic index pointing at them. No rule changed; a script checked the moved text plus the kept text reproduce the original exactly.
