Rename request_new_tab.sh to request-new-tab.sh to deal with changes in how IL deals with the _ character. Fix all references to request_new_tab accordingly.

Update all calls to UNIX-GETENV that involve a environment variable that is an underscore to use the new underscore character in Medley.

Put temp fix in for error due to wrong filkename for Notecards Documentation button. Error is actually in APPS-INIT in Medley. But putting fix in ONLINE-INIT as well to simplify testing.
