v0.3.2 — diagnostics attributes

Surface error / last_action_ok / last_start_ok / poll health as media_player entity attributes (no debug logging needed), and promote playback-start stalls and poll errors to warnings. Includes v0.3.1 command pacing + optimistic skip and v0.3.0 optimistic state.

Update via HACS, then restart Home Assistant.
