Both stubs return the expected default directly. Neither test establishes that the configuration route is absent, so neither verifies the stated missing-value case. Use an unconfigured document fixture, or a mock answer that models an absent route and returns the supplied fallback.

Make the offline persistence path observable.
RewardDistributionGranter.queueForOffline() calls Database.getConnection() inside the submitted task. This mock does not stub that call, so the task cannot persist a pending reward. Both offline tests still pass because they check only that no immediate grant occurred. Stub the JDBC calls and assert the saved pending reward after the task completes; a deterministic executor would make that assertion reliable. Mockito documents that unstubbed calls can return null. Close the executor after each offline-path test.
Each offline-path test creates a real ThreadPoolExecutor, and RewardDistributionGranter submits work to it. The test profile skips the production database shutdown path, so the executor remains active. Store it in the fixture and call shutdownNow() in @AfterEach.

IMPORTANT (testing): These two offline-path tests never stub Database.getConnection() (defaults to null) and submit the real async work to a real ThreadPoolExecutor that the test never waits on or verifies. The only thing actually asserted is that TrackingRewardType.GRANTED_TO stays empty — which is trivially true because queueForOffline() never calls rewardGranter.grantToOnlinePlayer(...) at all, online or not. The test would pass identically even if queueForOffline()/PendingRewardDAO.savePendingReward were completely broken or deleted, so it doesn't actually cover the offline-queuing behavior it's named for.
Fix: stub Database.getConnection() to return a mocked Connection, and either run the executor's submitted task synchronously (e.g. doAnswer(inv -> { ((Runnable) inv.getArgument(0)).run(); return null; }).when(executor).submit(any(Runnable.class)) via a mocked ThreadPoolExecutor) or otherwise verify the DAO interaction actually happened for the offline player.
Confirmed against RewardDistributionGranter.queueForOffline (src/main/java/us/eunoians/mcrpg/quest/board/distribution/RewardDistributionGranter.java:82-122).

IMPORTANT (testing): All four new resolveActiveQuestUUIDs tests only exercise the case where the target player is online (server.addPlayer()). None cover the onlinePlayer == null branch, which delegates to checkOfflinePermission() (PermissionQuestScopeProvider.java:100-110) — itself with two more untested branches (Vault plugin not enabled, no Permission service registered). A regression in offline permission resolution (e.g. always granting, or throwing) would go undetected.
Fix: add a test with no player registered on the server (offline UUID) asserting the quest is excluded when Vault isn't present, and ideally one with a mocked RegisteredServiceProvider<Permission> registered via Bukkit.getServicesManager() asserting the offline-permission grant path works.
