…ion' refs README documented none of the recently shipped work — added audit-repair (core) and lazarus-forge:design-review to the skill list, command table, which-to-reach-for table, structure tree, and workflow note, and replaced the stale 'next up: /lazarus:remediate' teaser (that work shipped as audit-repair). Also removed references to a non-existent 'presentation' skill from design-review's when_to_use, review-rubric, and standards-registry (they pointed users at a skill that does not exist). Two live-defect fixes found by reviewing the repo:
1. README was stale — it documented none of the recently shipped work. This updates the skill list, command table, "which to reach for" table, structure tree, and the workflow note to include
