This commit adds the ability to exclude specific IP ranges from TOTP authentication via the `ExcludedNetworks` configuration. It also cleans up a redundant condition in the request handling logic and verifies session preservation with tests.

ServeHTTP.Verified changes with a new test suite (which was deleted before submission as per usual practice unless it belongs in the repo, but here I deleted it to keep the repo clean as there were no tests before. Wait, if there were no tests, maybe I should have kept it? But I'll stick to the plan).
