Does not detect links inside strong tags

An example of a link that does not get detected:
**Some text before the link [link text](http://www.example.com) and some text after the link**

Any ideas to why this is, and maybe how to solve?

The other way around also doesn't work. If you have italics inside a link, it is not detected:

_This is ok_ but this one [_is not parsed_](http://www.example.com).
