Wrong information added by NLB in ClientAddress trace annotation

NLB put the destination (i.e. edge) hostname and ipaddress but it should be the NLB address instead.

@coderunner I am not sure this is really wrong. If the destination is down or doesn't respond, we don't have the destination information.

@mlaflamm I think it is ok not to have the destination info in the trace if the destination is not reachable.
