Enable Issue Templates

Currently, the GitHub page of the app is quite disorganized. There are multiple instances of the same issue, and the issues and discussions tab is being used inappropriately.

For example, we could have fields like this:

• Device and system name
• App version
• Description of the issue
• Steps to reproduce this issue
• Checkboxes:
• I have searched for similar issues, and none have been found
• This is an issue, not a discussion, feature request, etc.
• I didn't read the checkboxes so I pressed every single one and didn't know I didn't have to press this one.
• I didn't read the checkboxes so I pressed every single one and didn't know i HAD to press this one.

Thanks for the great suggestion! This will heavily clean up our issue tracker. I will set up the template files based on your schema later.
