docs(CHANGES): Refresh release notes

why: The changelog had drifted across several release-note styles, making recent product changes harder to scan and older entries harder to trust as user-facing history.

what:

- Rewrite CHANGES into the current release-note structure

- Add release lead paragraphs and user-facing deliverable prose

- Link current CLI, config, and API docs where targets resolve
