Description UX improvements

1. allow removing description when cursor is in description field (currently, command palette option requires the cursor to be in the node itself)
2. upon adding a description, put the cursor in the description input
3. only allow removing description via Remove Description command palette option
4. don't automatically remove an empty, newly-created description (possibly this is the same as 3)
