fix: disable automatic image insertion on active line

- Remove watcher auto-capture path that inserted /img immediately after copy
- Keep [📋] as non-intrusive indicator only
- Preserve explicit insertion flow (/img, Ctrl+V when terminal forwards it)
- Keep sequence-based dismissal logic so deleting token hides stale clipboard state
- Supports multiple images per line via repeated explicit /img insertions
