Fix Peaceman well model coupling in two phase flow app

Incorporate the peaceman equation to the Pressure equation by setting q = IP (P-Pwf) in the sink/source term. The user can set Pwf or q, in case of set q, the correspondent Pwf will be calculated by using actual time step well-block pressure.
