[Bug]: If an agent times out, don't complete assigned tasks

Bug Description

Running with a rate limited cloud agent, the agent may return an error message like:
"Permission denied: Reached overall message rate limit. Please try again later. Your limit will reset in 6 minutes. ...."

do not complete assigned work in that case - it hasn't completet, the agent just timed out

Steps to Reproduce

use pi with windsurf adapter and a model like swe-1.6.
running for roughly 1h, the agent runs into a rate limit

Expected Behavior

leave work as incomplete

Actual Behavior

marks tasks and goals as done (completed)
