I think the use of 'COFE Header' within the simplified comparison of CDA accreditation level 1 to FHIR Document Reference Level 1 may be very misleading to developers. I appreciate that a level 1 FHIR Document supports the same 'left hand side ' metadata reference extensions, however the CDA On FHIR Document Header Composition (COFE-Composition-1) structure is not used as the framework (primary resource) for Level 1 FHIR Documents and there is no mention of 'COFE Header' in the FHIR Document Reference Pattern.

On a similar vein - there is no explanation as to what the 'CDA left hand side' is within the Figure 12 Comparing Hl7v3 and FHIR Document Accreditation levels diagram (and the specification as a whole). Any implementer/ developer coming to this from a non-CDA background would struggle to understand this.
