Restore current-head Codex reviews for Review gate

Problem

Tools now has a protected Review gate status, but current-head chatgpt-codex-connector reviews are not being created for active PRs even after pushes, formal owner review comments, and Review gate reruns. The gate is therefore correctly blocking merges, but the external review producer is not supplying the review object the gate requires.

• Review gate reruns still fail with missing current-head review because only owner COMMENTED reviews exist on the current heads.

Acceptance criteria

• Determine why chatgpt-codex-connector reviews are not produced for Tools PRs.
• Restore the expected external review flow or document the exact manual trigger that creates a current-head chatgpt-codex-connector review.
• Keep Review gate required; do not replace it with owner self-review.
• After the trigger is fixed, rerun Review gate for chore: group compatible toolchain updates #54 and ci: smoke Sanchika docs route on deploy #53 and confirm the Review gate status passes when no unresolved review threads or requested-changes reviews exist.
