Support for structured content

Return and output schema so that agent that get the response without text parsing.

SHOULD also or MUST also?
because returning both might bloat the context.

MUST in my opinion. Last time I was testing with opencode, the structured content was ignored and the agent was considering the MCP response empty.

I don't think it's an issue for the context, agent that understands schemas, will just ignore the text content if structured content is present.
